Summary


We are looking to recruit a Nurse into the skill mix of the team, it will be an essential part of the role to support titration and monitoring of medication and monitor physical health observations within our Diagnostic Service. The successful candidates would be expected currently to work across 2 bases which are in Fleetwood and Lytham and would include travelling between as well as covering the Fylde Coast when visiting service users / carer’s etc.

This is an exciting opportunity and the successful candidates will be part of an evolving service that has the needs of the older adult population at its heart; and who are enthusiastic to come and join a forward thinking, dynamic and constantly developing team.

The post holder must have proven clinical skills along with excellent communication skills, good knowledge and understanding of dementia and the needs of individuals diagnosed with this condition. For this position it is essential that you have post qualification experience and be able to demonstrate a high degree of clinical skills and knowledge. The team offers high quality assessment and diagnosis both in a community and clinic setting, intervention and treatment alongside networking with other agencies to promote well-being and support for individuals and their carer’s and families during the process.





1) Be responsible for a small caseload of patients as lead professional.

2) To oversee medication titrations and medication monitoring

3) To liaise with GP regarding medication changes and ensure medications are supplied.

4) To fully participate in the continuous improvement of the service and the implementation of the operational and clinical policies.

5) Promote opportunities for local service development.

6) Professional Conduct, Professional Standards and National, Trust and local Policies and Procedures.

7) Demonstrate the sharing of knowledge, skills and attitudes to others.

8) Participate in multi-disciplinary meetings with the aim of improving clinical practice.

9) Must be willing to travel across the Blackpool, Fylde and Wyre to undertake home visits, this will require the use of a car- travel expenses will be paid.

10)Must demonstrate an understanding of our trust values.
