Summary

RESPONSIBILITIES Clinical To be professionally and legally responsible for all aspects of your own work as an autonomous practitioner, including the management of patients in your care. To be responsible for the provision of specialist physiotherapy care for patients in the community. To ensure patients under your management receive a high standard of clinical care and support other physiotherapy staff to do likewise. To accept clinical responsibility for a designated caseload of patients with a wide range of impairments, providing high quality, responsive specialist physiotherapy services within clients homes and other community locations as appropriate. To organise this effectively and efficiently with regard to clinical and management priorities and use of time. To be responsible for the care management of patients and the transferring of patients ongoing care to other physiotherapists or other professions / agencies, as appropriate, by providing comprehensive assessment; treatment plans and undertaking joint treatment sessions as indicated. To gain valid, informed consent from patients using the standards and guidelines provided by the Chartered Society of Physiotherapists (CSP professional body for Chartered Physiotherapists) and HCPC code of conduct. To work within the legal framework, seeking appropriate professional assistance when the capacity to consent is in doubt. To be part of the 2 day urgent response and support patients that are at risk of being admitted to an acute setting To undertake comprehensive assessment of patients, including those with highly complex presentation, using investigative and analytical skills. To formulate individualised management and treatment plans, using clinical reasoning, knowledge of evidence-based practice and utilising a wide range of treatment skills and options to form a specialist programme of care in agreement with the patient and/or carer. To use highly developed specialist manual physiotherapy skills, including dexterity, co-ordination and sensory skills, in the assessment and treatment of patients and to continually reassess and re-evaluate patient presentation altering treatment programme accordingly. To provide highly comprehensive immediate and planned advice, teaching and instruction to relatives, carers, other physiotherapy staff, other MDT members, homecare and agency support staff and other professionals to promote understanding of the aims of specialist physiotherapy for patients in the community and to ensure a consistent approach to patient care. To communicate effectively, highly complex and sensitive information, with patients, carers, guardians and medical practitioners/other professionals to maximise rehabilitation potential and to ensure understanding of their condition. Communication skills of persuasion, motivation, and explanation will be necessary with all patients. To maintain, and ensure all staff maintains accurate, comprehensive and up to date documentation, in line with professional standards, trust guidelines and legal requirements. To communicate assessment, treatment and results of treatment to the appropriate disciplines in the form of reports and letters including timely discharge summaries for other agencies including GPs, social services and other professional colleagues. To ensure clinical governance and risk management are integral to the service including consideration of tasks given to support staff and home care reablement assistants and find effective and efficient ways to solve problems and report findings to relevant departments. To assist in the personal care of patients undergoing treatment where necessary including dressing and toileting, participating in functional approach to rehabilitation. To be an active participant at MDT meetings including those based in GP practices, as required, working closely with the team on joint goals, reviewing patient treatments, progress and plan future rehabilitation / discharge from the service. To use clinical experience and knowledge to act as a resource for the team when analysing and judging the appropriate treatment pathway for highly complex patients. To communicate effectively and work collaboratively with medical, non-medical, nursing and therapy colleagues and other agencies across health / social services to ensure delivery of a co-ordinated multidisciplinary service. This will include case conferences and discharge planning and should ensure that all highly complex patient information is managed professionally with consideration for patient confidentiality and in line with data protection guidelines. To be responsible for equipment used in carrying out physiotherapy duties, and to adhere to departmental policy, including competence to use equipment and to ensure the safe use of equipment by others through teaching, training and supervision of practice. To maintain a high level of professional responsibility, by following professional/clinical guidelines/ standards, and Trust policies/procedures, and ensure that staff does likewise. To accurately report and investigate incidents (Datix) in line with Trust policy and standard operating procedures. Organisational To contribute to the development of Integrated Community Services within Berkshire West and, working closely with the Integrated Community Team managers and other members of the team, to be actively involved in the development of the physiotherapy service in the community for Berkshire West , using expert clinical reasoning skills; clinical research; audit and data collection to provide support and advice for physiotherapy and other staff within and outside the organisation, working with other Community Specialists, Primary Care teams and Social Services in conjunction with physiotherapy professional lead. To undertake the measurement and evaluation of your work and current practices using evidence based practice projects, audit and outcome measures, either individually, with other community specialists or with other physiotherapists. To make recommendations for change and be responsible for implementing changes within management of patients, setting and monitoring standards of practice. To be a resource for Integrated Community Service in Berkshire West , assisting in the promotion of improved care for Patients in the Community by participation in the collaborative working with other Community specialists, professional and charitable organisations. To lead on and work with the Team Managers to prioritise and allocate patients from the Reablement ,2 day pathway , Falls and Domiciliary Physiotherapy caseloads. To help review the caseloads ensuring effective use of available resources. To support the development of the Multi Therapy Assistants to support patients on all of the Therapy led patient pathways. To decide priorities for own work area, balancing other patient related and professional demands, and ensure that these remain in accordance with those of the Integrated Community Services and physiotherapy profession as a whole. To comply with organisational and departmental policies and procedures and to be involved in the reviewing and updating of processes as appropriate. To be actively involved in the collection of appropriate data and statistics for the use of the department. To ensure that all accidents and near misses are reported through the correct channels, informing the Community Team Managers and Leads as appropriate. Education and Training To share responsibility for training, supervising, clinical development and performance management of more junior physiotherapists, physiotherapy students and other staff to ensure cascading of specialist knowledge and skills. Provide day to day management and support for the band 5 rotational post within Berkshire West Integrated Services To assist in the provision of high-quality learning placements for physiotherapy students within the integrated Community physiotherapy service. To attend any mandatory training and be responsible for maintaining full compliance with all areas of training needs / requirements To be involved in preparing and leading Multidisciplinary training as required. Professional To abide by the CSP Rules of Professional Conduct and Core Standards. To maintain own clinical Continuing Professional Development (CPD) by keeping abreast of any new trends and developments, incorporating them as necessary into your work and keeping a record of activity in a CPD portfolio To be an active participant in service and professional development and training initiatives by the attendance and presentation at Physiotherapy trust wide forums staff meetings, tutorials, training sessions, external courses and reflective practice. To participate in the staff appraisal scheme and Personal Development Plan (PDP), as both appraiser and appraise. To attend relevant physiotherapy and CBNRT meetings. To share ideas and good practice, with the objective of continually looking at ways to improve the service. To lead on the implementation of new initiatives developed for community physiotherapy. To undertake any other duties that might be considered appropriate by the Community Team Manager.
