Summary
We are recruiting to a new role as a Health coach to work across one of our Community Integrated Mental Health Services (CIMHS) and a Primary care network (PCN) in Newham. The purpose of this role is to support people to improve the way they manage their physical health when living with a mental health condition, based on their context and what is important to them. You will work with a small group of service users to develop their knowledge, skills and confidence in managing their physical health and support them making changes in health-related behaviours.
You will work with people in the community, often at their homes and community venues. This is not a signposting role – you will need to be able to meet people where they are, physically and psychologically.
As this is a new role, you will be expected to share learning through case studies, data and themes emerging. As such, the successful candidate must be able to draw on qualitative and quantitative data.
The successful candidate will be kind, reflective and self-aware and will have a range of skills, knowledge and experience to draw on. You must hold a firm belief that people have untapped resources within them, that can be unleashed by providing high quality, non-judgmental support.
Please look carefully at the job description to ensure you evidenceclearlyin your supporting statement on how you meet the person specification. You will not be shortlisted without this being clear.
