This is an exciting opportunity to provide project management support to a new project focusing on supporting Adolescents and Young Adults as they transition from paediatric through to adult healthcare. This is an excellent time to join the team at WLCH, as transitioning well to adult healthcare is one of our key transformation themes, and is also identified in the quality priorities at both of our host trusts.

There are 10 million young people / young adults aged between 10 and 24 in England, who make up 17.7% of the population (DoH 2023). Ensuring that young people/adults are supported appropriately through their healthcare journey can have significant impacts including:

- improving access to services for young people

- improving health and wellbeing, and supporting behavior change

- reducing health inequalities

- reducing financial costs for the future

It is therefore important young people are confident to access services and are involved in discussions and decisions about their own health and therefore empowered to engage with health service as they become young adults.

Promote, develop and engage colleagues both internally and externally utilising various engagement platforms

Act as a champion for Young People (YP) providing leadership and working across all professional groups

Set-up and maintain an up-to-date project documentation for the Programme

Produce reports for internal committees and meetings

Establish links with existing Trust structures that can support the project goals (e.g. transitions leads, data teams, other clinical teams)

Working with transition leads (where in place) or paediatric leads, oversee and support benchmarking activities against the national framework provided by NHS England and project objectives to ensure alignment and adherence to best practices.

Contribute to the continued development and improvement of culture within the system.

Assist in the writing, updating and implementation of policies and procedures
