• We have an exciting opportunity for an experienced and enthusiastic registeredChildren's Nurse to join our Children's Community Nursing Team (CCNT).
• As aChildren's Community Nurse (CCN), you will play a crucial role in delivering high quality nursing care to children in their home environments.
• Your primary responsibilities will include conducting assessments, developing care plans, implementing nursing interventions, and ensuring proper documentation and follow up.

• Your primary responsibilities will include conducting assessments, developing care plans, implementing nursing interventions, and ensuring proper documentation and follow-up.
• To maintain a professional attitude and work in accordance with the NMC Code of Conduct at all times acting as a good role model.
• To be aware of own learning needs and limitations and to maintain professional expertise.
• To work as an autonomous and lone specialist practitioner.
• Support and deal with patients, parents, relatives and staff in stressful situations and recognising the need for counselling and refer on as appropriate.
• Empower patients and carers to enable them to safely care for their child within the community environment through education, support and risk assessment.
• Ensure prescribed medical care is carried out in accordance with medical request including administration of SC / IV / central line IV treatments, taking of blood samples as required.
• To support complex discharges transitioning to community setting , ensuring detailed communication with discharging hospital and safe discharge for child and family.
• To ensure all nursing procedures are carried out in accordance with trust policies.
• Deputise for team leader in their absence.
• To take appropriate action to address unexpected changes and situations, informing the team leader or community matron.

With nearly 9,000 dedicated staff caring for patients around the clock, they are the largest healthcare provider in southwest London.

Their main site, St George’s Hospital in Tooting – one of the country’s principal teaching hospitals – is shared with St George’s, University of London, which trains medical students and carries out advanced medical research. St George’s Hospital also hosts the St George’s, University of London and Kingston University Faculty of Health and Social Care Sciences, which is responsible for training a wide range of healthcare professionals from across the region.

As well as acute hospital services, they provide a wide variety of specialist care and a full range of community services to patients of all ages following integration with Community Services Wandsworth in 2010.

St George’s University Hospitals NHS Foundation Trust serves a population of 1.3 million across southwest London. A large number of services, such as cardiothoracic medicine and surgery, neurosciences and renal transplantation, also cover significant populations from Surrey and Sussex, totalling around 3.5 million people.

The trust also provides care for patients from a larger catchment area in southeast England, for specialties such as complex pelvic trauma. Other services treat patients from all over the country, such as family HIV care and bone marrow transplantation for non-cancer diseases. The trust also provides a nationwide state-of-the-art endoscopy training centre.
