This is an exciting and unique opportunity for an anaesthesia trainee with an interest in perioperative care to develop as a clinician and from a research perspective. The post is designed to provide a wide range of perioperative experience for a trainee, including provision of perioperative care and anaesthesia and working with a range of specialities to continue to develop perioperative services and research within the Trust. There will also be the opportunity to attend appropriate regional and national events. It is anticipated that during this twelve-month post the trainee will be able to amass the perioperative skills and competencies necessary for their specialist training.
Clinical duties will be 12-months in anaesthesia (obstetric anaesthesia on-calls).
There may be an opportunity for 6-months in anaesthesia (obstetric anaesthesia on-calls) followed by 6-months on the general theatres anaesthesia on-call rota.
The post can either be taken as a stand-alone role or as an Out of Programme Experience (OOPE) from a specialist training programme. For the latter, prior agreement must be sought with the relevant Specialty Training Committee (STC) and the deanery.
Main responsibilities:
• To develop perioperative pathways and services, and take part in departmental research activities.
• To provide educational opportunities for preassessment staff
• To further develop links with perioperative services and other specialities
• To participate in relevant research, management and educational governance
The post will be split 80/20 between clinical duties and perioperative care related activities, with the appointee taking part in research, service development and departmental education. The post-holder will be appointed non-clinical time to complete research, service change, quality improvement and educational activities of approximately 1-day per week (after annual leave and study leave), therefore approximately 40-days over the 12-months.
The hospital provides a full range of acute services for the local populations and a range of specialist services including perinatal (level 3) NICU and high-risk obstetrics, bariatric and laparoscopic surgery, stroke and neuro-rehabilitation. The local population and areas served by Homerton Healthcare NHS Foundation Trust are categorised by a striking variety of needs. Hackney is one of the most urban and cosmopolitan areas in the country. There are substantial immigrant and migrant populations and it is therefore very diverse. Over 20 languages are spoken and ethnic minority communities constitute over 50% of the population. Hackney’s mobile population of 250,000 people with expanding young multi-ethnic communities and high birth rates are characteristic of inner city living and give rise to distinctive health challenges.
Hackney is a markedly mixed inner city environment where poor housing and limited employment opportunities, which are known to have a negative impact on health, sit side by side with an influx of newer, more affluent residents. There is a thriving arts community and the area boasts some of the best parks in London.
Duties & Responsibilities
The role of the fellow will be to support the current perioperative pathways and develop new services and operating procedures. This will include running a pre-assessment clinic, working with staff in pre-assessment to help them develop their skills as well as providing anaesthesia on relevant theatre lists eg colorectal surgery. The departments also employ fellows in bariatrics, obstetrics and education and it is anticipated that there will be a degree of overlap between some of the work being done in order to further integrate services.

Learning and educational objectives:
In keeping with the Royal College of Anaesthetists, Stage 2 Level Curriculum, the post-holder will complete the following objectives during the fellowship:
· Deliver high quality preoperative assessment, investigation and management of patients presenting for elective and emergency surgery
· Deliver high quality individualised anaesthetic care to all patients, focusing on optimising patients experience and outcome.
· To plan and implement high quality individualised post-operative care for all patients.
· To take a leadership role in the multidisciplinary team in delivering perioperative care.
