Job Summary The post holder will be an autonomous practitioner who will be expected to take an active role in the team, respond, stabilise and make safe an element of acute care pathways by delivery of advanced assessment, and the management and treatment of patients over 65 years of age referred to the PCN Frailty Team and care/nursing home residents. The post holder will extend to the community to support people in their own home/extra care housing, undertake comprehensive geriatric assessment and advanced care planning for residents new to the care home setting or those with advanced frailty living at home/extra care housing. Have a key role in supporting the work of PCN in transforming the care of older, frail and more dependant patients, and will support the practices meet the requirements of the care home DES. The Advanced Practitioner role within primary care will provide comprehensive healthcare sensitive to the needs of the population, including advanced clinical assessments, history taking, diagnosis, plan and deliver treatment, prescribe, and evaluate. Working as part of a multidisciplinary team and across the primary, secondary, and social care interfaces in St Helens, the post holder will be responsible for co-ordinating the delivery of urgent health care needs of people at risk of hospitalisation or requiring a facilitated timely discharge and providing proactive advanced assessment of the practice population. This includes direct referrals from GPs, Care homes, and proactive post discharge reviews. The post holder will provide highly specialised health assessments and input as an independent and autonomous practitioner developing and leading relationships with members of the multi-professional team allowing for consultation and collaboration in a variety of health care settings. The focus will be to provide ongoing medical support and prevent unnecessary acute hospital admissions and enable patients to remain safely and supported within their own homes/nursing and residential homes. To assist patients to proactively manage and anticipate their future health needs and develop appropriate plans, in conjunction with their GP and other medical staff as well as Allied Health Professionals, Nursing and Social Care staff to maximise their independence and develop health plans to manage future health problems and support self-care. The post holder will lead in developing, delivering, and evaluating high quality patient centred interventions in conjunction with the associated clinical teams to ensure this is incorporated within both the overarching PCN Frailty Service and the St Helens Local Care Organisations objectives.
