Crynodeb
We are looking for someone who has a nursing or allied health professional background who can show evidence of community based post qualification experience of working with informal carers. You will need to have a good understanding of the role of the informal carer and be someone who can enable carers to understand the importance of their own health and wellbeing needs as well as those of the person they are caring for. You will be expected to work closely with Community Nursing Teams; Proactive Care Multi-Disciplinary Teams; G.P.s & Primary Care Teams and other health and social care professionals along with other charitable and voluntary services in order to ensure a seamless and equitable service for carers. Applicants will need to demonstrate a willingness to work flexibly and co-operatively with colleagues within the Carers Health Team and wider multidisciplinary teams across the whole SCFT area.As the team work over the whole of the SCFT area, staff must be willing to travel within this area.
