Maintain the safety of children and young people by ensuring a robust complex needs health care plan are in place. Devising care packages that have been commissioned by the Integrated Care System (ICS). To assess, plan, implement and evaluate the nursing care and support requirements for children with complex health needs. To engage with children and their parents/carers along with other agencies who are involved in their care to promote a person-centred outcomes approach to supporting their needs. To provide support to training and competency assessments and ongoing clinical supervision for Health Care Support workers/Administrative Clinical Assistants within the CCNT. To provide theoretical training and competency-based assessments in relation to nursing interventions for staff working in partner agencies including schools (mainstream and special needs), colleges and early years settings. Ongoing reviewing, critiquing and improvement of the training that the service provides both internally and externally. Undertake Nurse in Charge responsibilities to triage day-to day communications and referrals alongside other Shift Leads within the service (Acute CCN and Specialist Nursing for Schools Service). To support the Lead Nurse in developing service improvements to ensure the health needs of the children and young people who use the service are met. Actively engaging in the development of processes and procedures within the complex care team. If you have a non-medical prescribing qualification, you will be expected to practice as a prescriber as part of this role.
