The post holder is a member of an integrated multi-agency learning disabilities team (Health and Social Services). The team provides a specialist service to adults and older people with learning disabilities in a range of community environments across Camden. Camden is a culturally diverse borough with many multi-ethnic, bilingual and multi-lingual speaking communities. These may provide potential barriers to communication which could affect delivery of client care and the post holder will need to have an awareness of, and sensitivity to, social, cultural and economic issues.
· Assess the health needs of people with learning disabilities, plan, implement the treatment and monitor care, providing highly specialist advice to services users, paid carers and family members
· Manage a caseload of service users with complex needs (physical and mental health
· Provide advice on assessments such as mental capacity and best interest
· Develop effective and collaborative working with health and social care, multiagency colleagues, families and carers, including identifying and meeting the training needs and providing specialist and nursing advice
· Incorporate the philosophy outlined in “Valuing People Now” and the principles embedded within the overarching structures as set within the Learning Disability Strategy, clinical governance framework and Disability Equality Scheme.
· Supporting CLDS social work colleagues to carry out Safeguarding investigations for people with learning disabilities.

· To manage a caseload of adults with learning disabilities living in the community who have complex health needs, acute and/or chronic conditions, undertaking face to face visits in the community
· To undertake comprehensive and holistic health assessments of people with learning disabilities with diverse and complex presentations, using investigative and analytical skills to formulate individualised care and treatment plans, using clinical reasoning and utilising a wide range of skills and options to formulate a specialised programme of care to ensure health is preserved and/or improved.
· To contribute to differential diagnosis and health monitoring processes on the basis of evidence from assessment, seeking advice as appropriate, evaluating patient progress, reassessing and altering care plans as required.
· To co-ordinate and lead multi-professional health input for clients on caseload with very complex needs, working within a Health Action planning and/or CPA framework. To provide specialist knowledge about health of people with learning disabilities to all other professionals involved, for example on the effect of one chronic condition on another, and the risk of further deterioration and possibility of future acute episodes due to learning disability.
· To undertake the work jointly with health colleagues to lead on assessments and co-ordinate the multi-disciplinary assessment of those service users eligible for continuing healthcare funding. To lead assessments and reviews and coordinate support packages for service users. Identifying suitable placements through negotiation with the Brokerage service.
· To support individuals who have a learning disability and their carers to navigate the wider NHS through direct support and advocacy at health appointments and specialist support on a 1:1 basis to ensure understanding and implementation of consultant recommendations.
· To effectively communicate with people with learning disabilities, where there are significant barriers to understanding. And whom may also have additional problems of dementia, visual/auditory impairment, memory loss, behavioural problems, mental illness, depression, aggression and significant cognitive impairment and autism using a range of verbal and non-verbal communication tools e.g., Makaton sign symbols, photographs, pictures and objects of reference.
