An exciting opportunity has arisen for a Band 5 Physiotherapist to join our innovative and friendly Derby City Community First Team (previously D2A team), providing physiotherapy services to a high standard to our local population in their own homes and local care homes.

We are a short term service, with the team working to prevent unnecessary hospital admissions and also to facilitate early discharge from hospital/care homes. You will work closely with the patient in identifying, planning and reviewing their rehabiliatation goals to promote independence and self care.

You will work in partnership with Community First team members, service users, Derbyshire Community Health Services staff, UHDB colleagues, Social Care partners General Practice and other agencies as necessary to provide effective evidenced based care to a defined population.

This will be achieved through their contribution to and support of: effective delegation, case load management, monitoring of care standards, clinical leadership and the performance management of team members.

To identify the physiotherapy needs of individuals and communities for our patients, To contribute to case management, assessing, planning, treating and evaluating using clinical reasoning.

To contribute to the development and implementation of Physiotherapy within integrated care pathways and business plans. To be an active member of the wider Physiotherapy workforce of Derbyshire Community Health Services NHS Trust and willing to work wherever the service requires.

To be responsible for maintaining competency to practice through maintenance of professional registration, Continuous Professional Development activities and to maintain a portfolio which reflects personal and professional development and evidence of learning beyond registration.



The Purpose and Activities of DCHS?

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people’s own homes and via virtual consultations.

During the pandemic response we have led the implementation of public vaccination centres in collaboration with partners across health and social care.
