Summary
Join us in revolutionising healthcare delivery with our Discharge Digitisation Transformation Programme. We recognise the challenges posed by siloed data, manual processes, and fragmented communication. Through this programme, we aim to harness the power of technology and cultural change to streamline discharge workflows, improve patient outcomes, and enhance the overall healthcare experience.
Key Challenges:
• Siloed Data:Rich, large datasets are scattered across various software systems, hindering collaboration and decision-making.
• Manual Processes:Time-consuming manual tasks, such as updating spreadsheets and collecting data with clipboards, lead to inefficiencies and errors.
• Fragmented Communication:Interconnected processes rely on email communication, resulting in confusion and delays in patient care.
Programme Objectives:
• Digitise Processes:Implement electronic solutions to streamline discharge workflows, including electronic whiteboards and digitised referral forms.
• Cultural Change:Drive cultural change to embed digital processes into everyday practice, ensuring long-lasting transformation.
Benefits:
• Increased Efficiency:Reduce time spent on manual tasks, freeing up clinical staff to focus on patient care and increasing the number of discharges per day.
• Improved Patient Experience:Ensure consistent information and plans for patients, leading to earlier discharge and better outcome
The Band 6 Data Analyst for Discharge Transformation will play a central role in data management activities and coordinating a portfolio of projects aimed at enhancing Discharge and Flow. The post holder will provide datasets, leveraging advanced analytics to streamline processes, enhance cross-organisational collaboration, and drive cultural change towards a unified approach to patient care within Barts Health.
