Please refer to the attached job description and person specification for further details on this vacancy. To be able to produce monthly report of the service activity and performance. To ensure that safeguarding is a key priority and that all safeguarding concerns are addressed in a timely manner. To review service users progress regularly and ensure that any proposed changes are agreed with the service user and/or their family/carers. To be skilled in communication, establishing and maintaining good relationship with service users, carers, members of the wider health and social care team or other agencies. Reflect on practice regularly and plan personal development in order to achieve growth development. Access supervision from the Integrated Hub Lead. To carry out any other duties as and when requested by the Management Team. To maintain confidentiality in all dealings with service users and their records. To support service users, relatives and other care imparting knowledge to promote positive health and safe use of available equipment. To have knowledge of and be able to effectively use available resources.
