Westbourne Medical Practice is one of the best Medicines Optimisation focussed practices in Dorset, led by the highly experienced Lead Clinical Pharmacist Steve Williams.
The Senior Pharmacist will lead on conducting Structured Medication Reviews (SMRs) for patients that are aligned to the National Medicine Optimisation opportunities for the NHS through Dorset Integrated Care Board (ICB). The reviews are set to help reduce medicine related harm and cost .
You will have opportunities to share learning and develop links with other specialist pharmacists and the wider Primary care teams across Dorset. You will also regularly participate in multidisciplinary meetings and network with other advanced clinical members of the team.
The post holder will ideally have a prescribing qualification.
We are looking for a dynamic, enthusiastic individual that has the drive to push the service forward, works well in a team and autonomously and supports the training and development of others. The post holder should be familiar with System leadership and ideally have completed the CPPE PCPEP programme.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
The Senior Pharmacist will undertake clinical medication reviews and patient facing clinics, working in partnership with the patient’s GP practice, nurses, care homes, community pharmacists, secondary care and the local authority.
Utilising their prescribing qualification, the role will support the medication needs of the patients as well as educate other healthcare professionals on prescribing and the effective use of medicines.
The successful applicant will need to be self-motivated and able to work on their own, whilst also integrating into a well-established team. You will have excellent organisational, interpersonal and communication skills together with a good working knowledge of primary care and the challenges facing medicines management in the new era.
Main duties include:
• Medicines optimisation working closely with practice management to deliver key prescribing objectives in structured medication review
• Care home clinical services
• Antimicrobial stewardship
• Personalised pharmaceutical care, transfer of care
• Integrated care with community services
• Prescribing safety and governance.
• Leadership to the growing pharmacy workforce
