Summary

Joining our diverse and friendly NHS Continuing Healthcare Team as a qualified Social Worker, you’ll work collaboratively with Health colleagues, support individuals with their onward care journey following a Discharge to Assess period from hospital. Undertaking assessments under the Care Act 2014 and for NHS CHC, you’ll be working in partnership with individuals and their families to move them to appropriate care provision following assessment.

Our newly qualified Social Workers are paid at £33,951 per annum, pro rata for the first year of employment.

With the relevant professional registration, you’ll have UK based Adults’ Social Work experience to ensure you have the necessary knowledge required to develop quickly into the role. Ideally you’ll have experience of working in the field of CHC but if not, you’ll demonstrate a strong desire and commitment to learn and develop your skills and knowledge in the NHS CHC arena. Inquisitive and with strong analytical and problem-solving skills, you’ll enjoy collaborating with colleagues to achieve successful outcomes within this dynamic multi-disciplinary environment.

We recognise the benefits hybrid working can bring, so you’ll be able to work remotely (with equipment provided), with occasional office-based working at any of our office locations. Your office base will be a Hampshire office closest to your home address, however countywide travel will be required for this role. You’ll be required to travel to various locations to attend face to face assessments, meetings and training. We are happy to consider flexible working hours but will need to balance work patterns with the business needs.


Travel Requirements
You will be required to travel to various locations within Hampshire and it is essential that you have a current drivers’ licence and access to a reliable vehicle on a regular basis.
