We are looking for Registered Nurses to join Moorfields Private in our Outpatient Centre. The successful applicant will be responsible for:
- Protecting and supporting the health of individual patients and clients.
- The assessment of care and health education needs.
- The development, implementation and evaluation of programmes of care including discharge planning for each patient.
To maintain accountability for assessing, planning, implementing and evaluating programmes of care within the framework of team/primary nursing.
To ensure safe discharge of patients.
To promote a patient focused multidisciplinary approach to care in collaboration with carers, health care professionals and other agencies. Help patients gain access to health and social care, information and support relevant to their needs.
To educate patients and their family/carers as required where a need has been identified.
To provide information which enables patients to make choices about adopting a healthier lifestyle.
To complete patient documentation correctly, appropriately and in a timely manner.
To maintain a safe ward/departmental environment and work with other members of the team to produce health care environments that are conducive to safe, therapeutic care and ethical practice.
To assist in establishing and monitoring protocols/care pathways.
To participate in the promotion and maintenance of effective communication.

We have various sites across London but this role will be based at our main London office - Moorfields Private Outpatient Centre. • To work according to the Nursing & Midwifery Council and relevant professional guidelines as a named nurse or key worker (with facilitation) for a defined group of patients and take responsibility for:-
• Protecting and supporting the health of individual patients and clients.
• The assessment of care and health education needs.
• The development, implementation and evaluation of program of care including discharge planning for each patient. To gain experience and skills in:-
• Clinical practice for Ophthalmic Private Outpatient service
• Facilitation and teaching for diagnostics ophthalmic test, procedures and evaluation
• Management (as appropriate)
• To work in accordance with the MEH Nursing Strategy
• To maintain effective communication.
