The Adult Nutrition & Dietetic Service, provides an integrated service across both our Hospital and Community sites meeting the needs of our patients across the whole of the Bolton population. We have been funded for 0.85W.T.E Band 7, 1.0 WTE Band 6 and 0.5WTE Band 4 to provide a reach in service to the Adult Diabetes Service in Bolton.We are looking to recruit a Band 7 Dietitian who wishes to lead the diabetes service. The successful candidates will be part of the Adult Community Dietetics Team based in the centre of Bolton, providing an in-reach service to the adult Diabetes MDT.
You will be reviewing patients in a number of settings including the Diabetes Centre, Royal Bolton Hospital and in their own homes. Providing support in maternity, young adult and pump clinics.The successful candidate will input into the diabetes MDT.
You will be an integral member of the Diabetes Service and take a major role in the delivery of diabetes structured education (DAFNE). Act as a Highly Specialist Dietetic Lead for adult Type 1 and Type 2 diabetes and the education of adults with diabetes.

Clinical Lead role in the assessment and treatment of patients within a speciality, having developed highly specialist knowledge of that clinical area, and being an autonomous clinician, maintaining a high standard of clinical practice
Maintain a clinical caseload of complex high-risk patients
To participate in audit/evaluation for adult diabetes and other projects as required by the DAFNE Programme
To provide education and training to the wider multidisciplinary team in matters concerning the nutritional and lifestyle management of patients with diabetes including those with complex presentation.
