Summary
The Diabetes and Endocrinology service at Bolton NHS FT currently have a vacancy for a Diabetes Specialist Nurse.
You will work as an autonomous, advanced practitioner supporting with the delivery of an expert service within the specialist area both from an inpatient and outpatient base. The post holder will support community patients from the Bolton Diabetes Centre as well as within the acute setting providing direct care to inpatients and working as part of the wider MDT The role carries educational responsibilities for patients and other health care professionals which will be delivered in one to one sessions and group environments. The post does involve working within a multidisciplinary team with acute and community colleagues.
You must possess excellent communication and interpersonal skills and can demonstrate good leadership qualities to ensure our patients receive care that is safe, responsive, effective, caring and well lead.

The Diabetes and Endocrinology service at Bolton NHS Foundation Trust currently have a vacancy for a Band 7 Diabetes Specialist Nurse.
The role requires the individual to work as an autonomous, advanced practitioner supporting with the delivery of an expert service within the specialist area both from an inpatient and outpatient base. Our new team member must be self-motivated, a team player who is forward thinking and is enthusiastic about the nursing profession. The post holder will support community patients from the Bolton Diabetes Centre as well as within the acute setting providing direct care to inpatients and working as part of the wider MDT to reduce a patient’s length of stay and to support a safe discharge. The role carries educational responsibilities for patients and other health care professionals which will be delivered in one to one sessions and group environments. The post does involve working within a multidisciplinary team with acute and community colleagues.
The position is for 37.5 hours per week (over a 7 day week) and the post holder must possess excellent communication and interpersonal skills and who can demonstrate good leadership qualities to ensure our patients receive care that is safe, responsive, effective, caring and well lead.
