The role will carry a case load of vulnerable homeless individuals and rough sleepers with complex needs who require additional support and an intensive casework intervention so that they access or accept housing and support in their home area and are linked with appropriate services to sustain the outcome.

The role will be based within the local hospital team within the Royal Free Hospital working dynamically with The Camden Homelessness Prevention Service, Routes off The Street, Camden & Islington Foundation Trust and other local health and care providers.

The role is intended to be solution focused and outcome centred, initiating contact with the client group while still occupying hospital beds and ensuring that the client’s needs are assessed prior to discharge in order to formulate robust and meaningful move on plans. This will require a high degree of flexibility, judgement and discretion and the officer will be making and influencing decisions in a fast paced multi agency setting. The role is being funded for up to a year to increase the flow and impact of hospital discharges in Camden. To this end the role will offer innovative solutions and test new ideas. Using a test and learn reflective approach it is intended that the role will identify opportunities for system and culture change.

· Provision of expert housing advice and housing options advice to vulnerable individuals in reference to relevant housing law especially with those with complex, clinical conditions.
· Supporting clients in making positive and safe choices regarding their friends and family in order to generate reconnection options tailored to each individuals’ circumstances.
· Advocating for clients and accompanying clients to meetings in their home area to secure appropriate housing
· Completing a move-on assessment for all cases referred to the IDT which are homeless of rough sleeping, focusing on achievable reconnection plans
· Manage a caseload of complex individuals and facilitating outcomes which support move on to more secure housing
· Recording, measuring, monitoring and reporting back on outcomes in line with targets outlined
· Ensuring drug treatment options are available to clients in their home area and that any existing
treatment offers are transferred
· Working collaboratively with mental health service providers to ensure treatment options are available for each client where this is appropriate, in their area of re-connection
· Offer training, skill building and constructive reflection to increase the capacity of IDT’s to manage the client group
· Working “empowered and enabled” principles and innovating, learning and sharing ideas and successful approaches with other teams and services

CNWL Transfer of Care Hub is a well-established NHS provider offering a wide range of integrated discharge support and planning to the diverse and multicultural population across NCL. In addition, we also have services within University College Hospital and St Pancras Hospital.

The service is committed to partnership working. Our main partners are acute care services, social care and the community and voluntary sectors.

The aim is to provide a service specific to homeless individuals within the Royal Free Hospital The role will include a range of activities addressing the individual health and well-being, the development of pathways into community services, and improve the overall social inequalities via;

• Health promotion

• General Health screening

• Engagement in Primary Care services and facilitate access to organised health care

The purpose of this role will be imperative in improving integrated pathways to prevent fragmented services that are efficient and effective. The role is specific to support those to improve discharge outcomes especially for those who are homeless and require a safe and supportive discharge destination.
