A Vacancy at South West London and St George's Mental Health NHS Trust.


The Trust is seeking a Specialty Doctor for the Community Rehabilitation Team for a fixed term period to cover some aspects of a Consultant Psychiatrists maternity leave.

The Community Rehabilitation team ensures that a rehabilitation approach is maximized to support South West London patients in a variety of settings including out of area inpatient rehabilitation units, high support residential placements, acute inpatient units and community teams.

The team has a key role in ensuring that placements are reviewed within a recovery model ensuring promotion of independence. It oversees placement transfers and stepped care, to ensure they are effected smoothly and promptly, to contribute towards more effective systems for the future planning and reviewing of clients in residential and nursing placements, reducing the need for out of area placements and supporting people locally within the South London Footprint.

A vital component of this exciting and fairly new team is the interface and support to the South London Partnership (SLP) Complex Care Programme and the team will work closely with the SLP team in a number of areas including:
• Managing step down from out of area inpatient rehabilitation units
• Ensuring high quality care is evident in referrals to the SLP Single Point of Access (SPA)
• Working with the SLP Clinical Assessment Team
• Supporting existing providers such as One Housing Group and Bridge24
• Supporting the flow of care through the system
• Maximizing community stakeholders including the voluntary sector to enable full recovery and independence for our users

The community rehabilitation team is part of the complex care pathways and works closely with the two rehabilitation inpatient units, Phoenix Unit (High Dependency) and Burntwood Villas (Community Rehabilitation). The team also provide the health input to the two high support placements in Wandsworth managed by the One Housing Group, Thrale Road and Westmoor House and oversee the clinical team which inputs to SLP funded community high rehabilitation support placements provided by Bridge24.

About our location:

Tolworth Hospital, Surbiton

To create the very best environment to care for our patients, this site’s layout will be transformed in 2026 to add a number of wards to the existing three. Tolworth has a canteen and is near shops, the A3, Tolworth station, and is a bus ride from Surbiton station, with fast connections to Clapham Junction and London Waterloo.
• To assess and treat service users in the community and in a clinic.
• To attend weekly IRH MDT meeting and to support the team with medical input as required.
• To arrange and participate in CPA meetings for service users.
• Undertake urgent psychiatric assessments when necessary.
• To liaise with other parts of the service and GPs and non-NHS organisations wherever applicable.
• Write up all their notes using the Trusts Electronic Record system RIO which also records time and date of entry. The post holder is personally responsible for entering their notes onto the system themselves.
• Ensure effective and timely documentation and communication of clinical decision making and management plans.
• The post holder is expected to have Section 12 approval.
• Compile reports for MHRT and attend on behalf of RC when this deemed appropriate (CTO patients).
• To take part in the discharge planning CPAs and advising the primary care colleagues.
