NCH&C is hiring several multidisciplinary roles within the state-of-the-art new Willow Therapy Unit. Opening at Norwich Community Hospital in 2024, the new facility will provide a supportive and comfortable environment for 48 patients.

The restorative environment at Willow Therapy Unit will enable patients to make steady, step-by-step improvements and gain independence as they prepare to return to the community. Patients will be supported to play an active role in their own care and feel confident about performing daily activities when they leave. Everything down to the room decor, layout, and furniture at Willow Therapy Unit is designed to help patients get home faster – there’s even a supervised kitchen where patients can make themselves drinks and snacks.

Healthcare will be delivered by a multidisciplinary team of clinical professionals dedicated to patient recovery and wellbeing.

As a Inpatient Staff Nurse, you must live by our values and use your skills to ensure the ethos becomes part of the culture within the team. Being able to communicate well with a variety of people is an absolute must.

• Be responsible for delivery of care in a reablement setting.
• To support an environment where all team members are responsible for a reablement ethos using a positive risk-taking approach for patients, their carers and families.
• To support patients being as independent as possible within their functional limits.
• To work as part of a multidisciplinary team within a quality framework to ensure that the highest standards of care are met.
• To support and deliver innovation within discharge practice and pathways.
• To work with system partners including the voluntary sector to ensure that patients experience a seamless transfer of care both into and onward from the unit.
• Be responsible for the delivery and co-ordination of care to a named group of patients.
• Work proactively within the team to achieve unit and trust goals and objectives.
• Work with and promote recognised policies, protocols, and guidelines.

Clinical/Professional
• Is wholly responsible for his/her practice in accordance with the Nursing and Midwifery Council code of professional conduct. Takes every reasonable opportunity to sustain and improve his/her knowledge and professional competence. To continue any development highlighted through the appraisal process and attend mandatory and specific training sessions.
• Plan and assess programs of care with realistic nursing and reablement goals with the consultation of patient, family and multidisciplinary team.
• To be sensitive to patients needs in respect of courtesy, dignity and Privacy.
• Promote effective communication with the multi-professional.
• Provide information and education, which enables patients to make choices about adopting a healthier lifestyle (in an appropriate manner to meet patients learning needs).
• Recommend innovative changes in clinical practice, to enhance patient care.
• Participate in audit processes and action outcomes.
• Demonstrate an understanding of research and application to clinical practice.
• Use clinical expertise and knowledge to promote reablement care. Taking on a ‘Link nurse’ role where appropriate.
• Ensure the safe control and custody of drugs within the ward/department in line with Trust Policy and local Standard Operating Procedures.
• Inform the Nurse or Therapist in Charge of changes in circumstances which may affect safe and competent practice.
• Ensure that all patient care is documented accurately and legible and that information systems are maintained in a timely manner.
• Work collaboratively with outside agencies.
• Be aware of and utilise other statutory and voluntary agencies that can provide appropriate care for patients.

Management/ Organisational Responsibilities
• Ensure effective and efficient use of all unit resources.
• Provide leadership to all members of the unit promoting effective teamwork.
• Be responsible for monitoring staffing levels and the skill mix to ensure that this is appropriate to meet the needs of patients and taking initial action to rectify any identified deficits through service Unit lead.
• Maintain a positive approach to achievement within the team.
• Ensure adequate provision and maintenance of equipment.
• Assist with the induction of new staff and students, acting as a mentor.
• Act as a role model giving guidance and support to relevant staff and students.
• Participate in problem solving related to work organisation.
• To ensure that patients and carers are aware of their current and ongoing health needs.
• To participate and promote clinical supervision.
