We have a unique and exciting opportunity to join the Upper GI Cancer team based at the RVI as a Cancer pathway coordinator.

Working alongside the existing post holder as a job share, the post holder will be responsible for ensuring patients progress through all aspects of the cancer pathway by coordinating their care and service delivery form referral to discharge

Working as an integral member of the MDT and under the guidance of a nurse specialist, the post holder will be a point of contact for patients, carers and the MDT and the role will involve direct contact with patients in the clinical environment, indirect care coordination and regular liaison with other departments and services.

Applicants must be enthusiastic and motivated with previous experience in health or social care setting ideally working with patients

• Triage incoming calls, using a risk assessment framework and initiate appropriate response according to protocols and individual pathways, using good communication skills, basic clinical awareness and appropriate tools and procedures, liaising as appropriate when non routine and refer complex decisions to the team for assessment and review.
• Document and monitor all aspects of care coordination and service delivery, supporting data collection for audit
• Coordinate the care for a defined group of patients assessed by a registered practitioner.
• Organise and prioritise the designated workload in relation to identified needs
• Coordinate the handover with other teams to facilitate safe and effective transition of care between services in order to provide seamless support for people
• Coach patients and carers on how to make contact when they feel that their condition or needs have changed, including what to do out of hours
