This is an exciting opening for a clinician (Physiotherapist or Nurse), with the right knowledge and skills, to be a Senior Practitioner in the Brighton and Hove Community Respiratory Service, alongside the Clinical Team Lead.

It is a great opportunity to provide management and leadership to a broad multidisciplinary team and across the range of services that we offer. These include Home Oxygen: assessment and review, Pulmonary Rehabilitation, Nebuliser loans, Rapid Response. We are looking for an experienced band 7, or an established band 6 who is looking to further their skills and career.

The successful applicant will support the team to deliver a high standard of care to patients in their own homes, care homes and clinic settings. The ideal candidate will have significant relevant respiratory experience, excellent communication skills, be passionate about empowering patients, and collaborate with others to deliver high quality, patient-centred care.

You will inspire, innovate, adapt and lead in changing healthcare environments, and be able to identify improvement strategies that will carry forward into the future. There will be an expectation that you will provide respiratory leadership across the East SCFT area.

You will also be prepared to undertake any additional education and training to support professional development.

assessment and prescription.

1. As an autonomous practitioner and using advanced respiratory skills, carry out assessments and provision of appropriate care to a specialist caseload of patients. Those referred to the integrated respiratory service will have significant comorbidities and may have social complexity.
2. Provide expert advice and support to patients to improve self-management within a home setting.
3. Appropriately triage, visit and treat patients who may be significantly unwell, and escalate medical input as necessary.
4. Provide expert advice and support to colleagues within the respiratory service.
5. Be an expert clinical resource in terms of patient advocacy and health system process knowledge.
6. Ensure an evidence based approach, and contribute to the development of the service through reflective practice, clinical supervision and audit.
7. Support the running of the Community Respiratory Service; to include the Home Oxygen Service, Pulmonary Rehabilitation Programme, Rapid response service and nebuliser loan service.
8. Provide specialist advice to other professionals both within the Trust and outside agencies, and to be able to effectively liaise with families and carers.
9. Provide clinical leadership to the team, with the ability to provide this support and leadership across the East SCFT footprint for respiratory services.
10. Contribute to service achievements and performance, against service plan, targets and standards, including inputting activity data in order to support service development and meet contractual requirements.
11. Assist the senior clinicians and Clinical Lead to maintain continuity of care anddeputising when appropriate.
12. Reporting and investigation of accidents, incidents or complaints.
