Summary


12 Month Fixed-Term Contract

A fantastic opportunity has arisen for a Diabetes Specialist Nurse to join our Peterborough Team working full time 37.5 hours per week. If you have compassion, empathy with the ability to work automatously this is the opportunity for you.

You will work as part of the multidisciplinary team with the aim of preventing admission and providing ongoing care for patients with Diabetes Disease within the community.

You will also be responsible for managing a caseload of patients with complex Diabetes needs, using evidence based practice and patient centred goals to assess plan implement and evaluate inventions

The role includes promoting better understanding of Diabetes for patients, their families and carers and other agencies involved with the patient through education, support and counselling.

Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. • To act as key worker as appropriate, co-ordinating multidisciplinary care
• To provide expert support, advice and education to raise awareness of the needs of patients with Diabetes
• To undertake all aspects of clinical duties as an autonomous practitioner with access to advice and support from senior colleagues
• To work closely with both primary and secondary care to ensure a safe and effective patient pathway is in place for all diabetes patients under the care of the community diabetes team

• Plan, implement and evaluate individualised specialist patient care programmes based on evidence based practice and the application of clinical reasoning skills
• Promote and support self-management and engage with patient support groups
• To be professionally and legally accountable for all aspects of own work including management of patients in your care.
• To organise the caseload efficiently and effectively with regard to clinical priorities and use of time
• Promote better understanding of Diabetes for patients, their families and carers and other agencies involved with the patient through education, support and counselling.
• To delegate care of patients, as appropriate, to assistant staff and supervise their work
• Recognise and act on signs, symptoms and complications of diabetes conditions
• To manage a caseload of patients with complex Diabetes needs, using evidence based practice and patient centred goals to assess plan implement and evaluate inventions
• To deliver accredited Type 2 (DESMOND) Structured Education to newly diagnosed patients
• To attend regular team meetings and other meetings as required.
