This job description is an outline of the main duties of the post. The post holder will be required to undertake other duties commensurate with the grade as directed.

As part of the Targeted Lung Health Check Programme, we are increasing the support we offer to people who are screened by us, to help them to stop smoking or to reduce harm from tobacco and to become smoke free.

The overall purpose of the Targeted Lung Health Check (TLHC) Programme is to enable the early detection of lung cancer to save lives. Coventry & Warwickshire Integrated Care Board (ICB) and the West Midlands Cancer Alliance (WMCA) has identified high disease and smoking prevalence areas within Coventry & Warwickshire where a significant impact can be made from detecting early-stage lung cancer.

This requires a high calibre individual who can work autonomously in a stimulating and challenging environment to achieve outcomes efficiently.

The post holder will focus on engaging with TLHC patients and offering them practical support to change their behaviour to achieve their own choices and goals. While supporting them to make positive health changes that will improve health outcomes

This post will contribute to tackling inequalities in health through promoting and supporting people to stop smoking. The post holder will primarily provide tobacco advice and support with potential future development to provide prophylactic therapies to individuals who want to stop smoking and reduce their harm from tobacco.

Specifically, the post holder will:



· Promote the general health and wellbeing of people who use the service through offering healthy lifestyle advice and support.

· Work with people to change their behaviour to improve their health.

· Act as a smoke free champion promoting the smoke free message through their day-to-day work.



· Ensure a seamless transition to the Stop Smoking Service.



· Work with minimal indirect supervision operating within the assessed level of competence and being accountable for all aspects of own work.

· Be a first point of contact to any patients that agree to a referral to the Stop Smoking Service

· To contribute to the monitoring and evaluation of the service, including the completion of the minimum data set requirement. This will include following up quitters who do not attend for planned support.

· Maintain accurate patient records in line with Trust guidelines.



· Respond effectively to referrals within stated timescales.



· Work in partnership with the Community Stop Smoking Service ensuring a smooth handover of patient care.



· Ensure administrative duties are carried out efficiently and effectively in order to meet the needs of the service.



· Take part in health promotion activities to signpost people to smoking cessation support.



· Contact all TLHC patients who are still smokers at their 24 months to offer another opportunity for smoking cessation support and reminder of forthcoming scan appointments.



Postholder would be required to add data onto clinical systems including Spectra, Soliton and EPR from previously acquired information.

Knowledge Skills and Experience Required:

· Training and knowledge on smoke-free pathways and the available support.



· Knowledge of the TLHC programme, its aims, and objectives.



· Ability to deliver stop smoking advice, including advice on stop smoking medications, working within recommended protocols and guidelines.



· A sound understanding of developing individually tailored support plans including recommending treatments based on assessed needs.



· Have experience in and be competent to participate in educating patients about the benefits of stopping smoking. Promote smoke-free homes and support and advice for patients about passive smoking.



· Maintain accurate up to date and comprehensive records at all times. Furthermore, to contribute to the monitoring and evaluation of the tobacco dependency service.



· Ability to have difficult but motivational conversations with patients about the risks of smoking.



· Good organisational skills.
