We have an exciting opportunity for Community Specialist Practitioner students to work in District Nursing within Manchester and Trafford Local Care Organisation.

This is a 53-week Level 7 post-graduate diploma (District Nursing). This would be a secondment for internal candidates or a fixed-term 53-week contract for external applicants. You will be paid at Band 5 for the duration of the course.

Our Teams work as one in the Neighbourhoods they support - delivering and designing services in partnership with local people. We also provide key services that work across wider localities and City wide. We believe that our pioneering approach will make a dramatic difference to the health of people in our City and we are looking for you to join our team and help us to do this.

The Community Specialist Practitioners’ course is designed to prepare practitioners for their role as a District Nurse, through meeting the Professional Standards for Specialist Practice as outlined by the NMC (2022).

In addition, from 2016 the QNI Voluntary Standards for District Nursing Education and Practice will be adopted.

This opportunity is in conjunction with Bolton University.

You will have a current first level registration on part one of the NMC Register and you will have at least 120 credit points at level 6 or APEL equivalent.

There are 16 posts across the City: North, Central and South Manchester and Trafford.

Manage an integrated caseload of patients within the Neighbourhood Team providing support and care to patients suffering with a varying complexity and intensity of needs, promoting health and well-being, self-care, and independence.

Promote clinical excellence and evidence-based approach to care by complying with the agreed Trust policies and procedures maintaining own clinical competencies and developing the competencies of others within a clinical leadership role.

Work towards developing the skills to support management of the clinical area alongside sisters within the team.

Monitor agreed induction, orientation and preceptorship programmes for newly appointed staff and students.

The role requires attendance at the University of Bolton and a Clinical Placement in a designated area, supported by a Clinical Supervisor and Assessor.

Ratio of University attendance to Clinical Practice is approximately 50: 50

Work towards developing the skills to support management of the clinical area alongside Caseload Holders within the team.

Take a supporting role in ensuring the compliance of quality assurance initiatives with agreed clinical standards.

Support teams in ensuring that full use is made of the ability to develop the role of the nurse in accordance with the requirements of clients, the service and the Nursing and Midwifery Council (NMC). Act as a role model demonstrating commitment to the Trust Values Framework.

Comply with requirements to submit information supporting clinical activity.

Comply with Employment Contract to complete both Academic and Clinical Practice Modules in agreed timescales.
