Exciting opportunities have arisen for a qualified School Nurse (SCPHN) to join our dynamic Young Greenwich Health team to a 12 month fixed term maternity cover role.

Young Greenwich is a new integrated programme commissioned by the Royal Borough of Greenwich. The programme is led by Oxleas and delivered with partners Charlton Athletic Community Trust (CACT) who together with METRO form theYoung Greenwich Partnership.

Young Greenwich will deliver seamlessly in and out of schools, youth hubs and other community venues with strong pathways for pupils in schools into community-based activities and support and vice versa.

The successful candidate needs to be highly motivated and enthusiastic delivering the healthy child programme and model of community health across schools in the south of Greenwich. We are looking for a qualified SCPHN: School Nurse that possess excellent communication skills and have a forward thinking approach to new ways of working within our partnership in youth hubs including attending evening sessions.

Youmustbe a registered nurse and hold theSCPHN: School Nurse pathway degreewith sexual health, safeguarding and emotional health knowledge desirable. Applicantsmusthave a current driving license with access to a car daily.

In return Oxleas will support the post with supervision, appropriate training and self-development opportunities and a network of peer support. Oxleas operate a family friendly policy.

Oxleas offers a wide range of NHS healthcare services to people in community and secure environment settings. Our services include community health care such as district nursing and speech and language therapy, care for people with learning disabilities and mental health care such as psychiatry, nursing and therapies. We are the largest NHS provider of prison health services providing healthcare to prisons within Devon, Dorset, Bristol, Wiltshire and Gloucestershire, Kent and South London. • To lead the delivery of the Healthy Child Programme, overseeing and participating in the delivery of the key touch-points / health and wellbeing reviews as part of the multi-disciplinary team
• To liaise effectively with colleagues within Young Greenwich and partner organisations to ensure care for children and families is timely, well-coordinated and effective
• To collate information and ensure appropriate referral for children with on-going health needs to relevant services
• Be proactive in the development, review and promotion of the Young Greenwich service
• To analyse health needs to inform the development of Young Greenwich strategies
• To work according to local and national safeguarding children policies and guidance, acting as a specialist health resource and contributing to strategy meetings, initial case conferences and other safeguarding meetings where a health need is identified and participate in the decision making process
• Demonstrate a clear understanding of own responsibilities and contribute to team working and development of partnership initiatives
