As a team, we are proud to be in a position to recruit a highly motivated and innovative practitioner into our successful School Nursing team as bank as and when to meet the needs of the service - hours will vary. The practitioner must be a Registered Nurse with an additional SCPHN degree in School Nursing to be able to fully understand the knowledge and skills this role commands.

You will be responsible for a defined caseload, demonstrating leadership skills to guide and drive the team forward. You will lead by example demonstrating evidenced based practice and professional values within your day to day work.

Doncaster has a diverse range of health need within the school aged population. It is for these reasons you will be expected to promote the physical and emotional well-being of children and young people to establish good health both now and in the future. You will promote early intervention and health promotion to strive towards reducing health inequalities across the borough of Doncaster, by referring to local and national public health data, tools and resources.

You will support and make referrals to appropriate professionals and agencies to safeguard a child who may be at risk of suffering significant harm working in partnership with the wider multi-disciplinary team to ensure the child reaches their full potential.

• Clinical Nursing Practice
• Assess the health needs of children, young people, their families and other carers and identify and initiate appropriate steps in accordance with active caseload management.
• Assess and manage clinical events to ensure safe and effective care.
• Act independently within a multidisciplinary / agency context with appropriate support from Team Leader as required.
• Provide therapeutic and educative interventions to individuals, families and communities.
• Plan and evaluate health promotion activities within schools and communities according to the school health profile.
• Initiate action to identify and minimise risk, with regard to child protection and safety, working in partnership with families and multi-agencies.
• Manage the Single Point of Contact.
• Our team offers an after school e clinic so there is an expectation that you will work flexibly to meet the needs of the service.
