Trafford Home Based Treatment Team has an exciting opportunity for a dynamic and enthusiastic individual to apply for this newly developed role, to improve the service delivery within theHBTT.

“The successful applicant will be attending the University of Salford for participation in the Trainee/Advanced practitioner course, commencing on the September 2024 intake.”

The successful candidate will play a central role in supporting the clinical and operational team to ensure high quality, recovery focussed care is delivered to our service users.

On completion of the training the post holder will be recognised as an expert practitioner in an identified field of practice, and will advise the MDT on expert, safe and effective models of care and treatment, (including non-medical prescribing) within the HBT setting leading to improved patient outcomes. They will be an integral part of the MDT supporting clinical decision making and supporting the service pathways, developing nurse led clinics and providing assessment, formulation, and brief interventions of care, including discharge planning within agreed protocols and procedures.

Trainee Advanced Practitioners will take up components of the role as competences are developed and are demonstrated in practice.

The post holder will effectively be required to work across the range of services as determined by service user need.

The post holder will provide advanced and expert level of practice (including non-medical prescribing) within the HBT leading to improved patient outcomes.

The post holder will support the clinical and operational team to deliver on service improvements in the quality of care and safety through service development, evaluation, and audit.

They will be an integral part of the MDT supporting clinical decision making and supporting the service pathways, developing nurse led clinics and providing brief interventions.

The Advanced Practitioner will hold a case load and will lead assessment, formulation and prescribing of care planning including discharge planning within agreed protocols and procedures.

The job holder may additionally lead on practice and service developments. The job holder is required to demonstrate clinical and practice expertise via interventions, supervision and modelling good practice.
