Job title: Social Prescribing Macmillan Team Manager Service: Macmillan Social Prescribers Service Salary: Grade 3 Point 24 - 28 Hours: 22.2 hours per week (part-time) Location: Hackney, London Responsible to: Service manager Summary of job: Working in partnership with Macmillan Cancer Support, we aim to develop interventions for people with cancer in the area and support the delivery of personalised care to everyone diagnosed with cancer. The role is to manage, lead and supervise staff of Macmillan Support Workers - Macmillan Social Prescribers and volunteers to a high standard. You will work holistically to plan, support and help individuals achieve the goals and outcomes. You will contribute to the ongoing development of team working and implementing of the new service, produce comprehensive regular outcome focused reports and manage patterns of work ensuring standards are maintained and improved. You will provide leadership and line management to support the delivery of a safe, effective and appropriate service. Key tasks and responsibilities: 1. To provide leadership, guidance, supervision and appraisal to the team of Macmillan Support Workers - Macmillan Social Prescribers and volunteers 2. To support workers in delivering targeted work with individuals to plan their support and achieve the goals and outcomes that are important to them, and to work with the wider team to support the development and implementation of an action plan to support the individual. 3. To make a commitment to regular supervision, including the ability to be responsive to critical challenge, advice, feedback and direction. 4. Act as safeguarding lead for the social prescribing service and work according to Family Actions and City and Hackneys safeguarding policies and procedures, safeguarding vulnerable adults and children in collaboration with statutory and voluntary agencies, escalating cases when required. 5. Provide high quality supervision to staff and volunteers, monitor the safeguarding responsibilities and training of staff and provide support and advice to staff around safeguarding issues and work closely with Service Manger on complex issues. 6. To establish and maintain effective liaison with stakeholders including health, voluntary, social and education resources, attending relevant meetings as necessary. 7. To work in partnership with all voluntary and community organisations to build a comprehensive database of local resources to design and support the Social Prescription Menu and ensure familiarisation of the support available from Macmillan Cancer Support. 8. To ensure information on sources of voluntary and community support is up to date at all times to enable effective and accurate signposting and linking of individuals with services. 9. To train and develop GPs and primary care health teams knowledge on how to identify patients suitable for social prescribing service referral on a quarterly basis. 10. To monitor outcome tools and analyse relevant data to support the Service Manager in delivering on KPIs as required. 11. To undertake holistic assessments and co-design a comprehensive Care and Support Plan with individual service users on an exception basis, identifying support needs to ensure maximum engagement in improving health and well-being. 12. To carry out case audits on a regular basis, complete audit reports and share information with the SP worker/s and the Hackney services manager. 13. Set up and maintain comprehensive data and evaluation systems, including individual health outcome tool (Wellbeing star and EQ5D). 14. Attend the steering group including representation from all stakeholder groups to support the on-going development, monitoring and evaluation of the programme. 15. Provide monthly and quarterly comprehensive outcome focused reports detailing the progress of the service, including budgets. 16. To keep records of your work and adhere to confidentiality, information sharing protocols and provide monitoring information as required. 17. To implement the principles of Family Action and Hackneys Equal Opportunities Policy in every aspect of your work and positively promote the principles of the policy amongst colleagues, service users and other members of the community. 18. To work flexibly as required by the service and to take part in Family Actions and other organisations meetings and events to promote, support and celebrate the work of the service and the agencies. 19. Contribute to the health and safety plan and quarterly reviews, monitor absence and sickness. 22. To ensure you have an understanding (appropriate to your role) of and comply with Family Actions procedures for promoting and safeguarding the welfare of children and vulnerable adults.
