ROLE SUMMARY You will undertake the comprehensive assessment of service users needs in the West Essex area. Assessing and treating your own specialist caseload of service users in accordance with Trust clinical pathways, you will provide effective, tailored, evidence based occupational therapy to service users. In doing so, using goal setting and interventions focused on re-engagement in self-care, productivity and leisure activities, and promoting health, wellbeing and independence. You will be expected to identify and use relevant occupational therapy standardised assessments and outcomes. You will also exchange complex sensitive information with service users and assess their response to activities in accordance with agreed models of practice. You will also provide occupational therapy advice to other disciplines and be involved in providing training, supervision and support to others as required. You may lead clinical audit and undertake research. You will provide leadership in the delivery of specialist occupational therapy interventions, and provide care coordination to caseloads. You will also have responsibility for the day to day supervision and management of lower banded staff within your professional group and team locality. This will include the management of these staff in line with agreed Trust HR processes. You will be expected to work collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services; providing a 7-day a week, 24-hours a day service for 365 days a year working shifts where appropriate and operationally required. In delivering your duties you are expected to display behaviours that are aligned with the Trust values and as required in accordance with the NHS Constitution at all times. KEY RESPONSIBILITIES CLINICAL Clinical Function Responsible for the initial assessment process, as well as observation and the on-going assessment, providing accurate and timely feedback Responsible for the planning, implementation / evaluation and on-going review of treatment plans. Provide formulation, delivery and review of individualised evidence based care / care packages according to NICE guidance including non-medication strategies. Organise and deliver therapeutic activities for service users. Assess service users response to activities in accordance with agreed models of practice e.g. MOHO Provide practical support, advice and education to service users, carers and other disciplines. Provide specialist OT advice on equipment, telecare etc. to other disciplines. Assess service users for access and allocation of equipment within their individual needs. Responsible for the recording of all patient related activity information onto System One. Lead induction of staff and fieldwork education of students. Produce accurate and timely documentation on all clients in line with your Professional Code of Conduct and Trust policies Carry out risk assessments including any potential hazards in the service users environment. Attend multi-disciplinary meetings i.e. referral / review / business. Report and document incidents as required. Develop and facilitate therapy groups such as Cognitive Stimulation Therapy, health promotion etc. Undertake carers assessments in line with Trusts key performance indicators. Be competent in use of relevant screening and assessment tools. Provide comprehensive assessment, care planning, treatment and review of care in line with the requirements of Mental Health Care Clusters. Participate in the continuing development of a high quality service that is underpinned by evidence based practice, care pathways / packages, NICE and DH guidance. Participate in audit and research where appropriate and contribute to the development of service policies. Able to undertake the role of care coordinator. Non-Clinical Function Promote Occupational Therapy within the service and wider community. Participate in the implementation of the OT Strategy. Take part in service development and review. Identify aspects of the service which need to be improved and implement appropriate changes in conjunction with the Clinical Manager, and Advanced OT Practitioner and Area Consultant OT, where appropriate. Participate in the recruitment process of OT and other clinical staff as required. Provide clinical and managerial leadership and expertise to team. Ensure the service meets the agreed performance / contractual targets and adheres to local and government initiatives. Work closely with and assist Clinical Manager / professional lead with development and implementation of team operational policies. Deliver training where appropriate for other professionals. Manage the OT capability process as appropriate.
