Summary


We are looking for dynamic individuals to work on an innovative project across the Elderly Care wards at Colchester Hospital, providing reablement to patients during there inpatient stay.

Successful applicants will work with suitable patients to enable them to maintain and improve their mobility and functional Independence.

You will work closely with other Therapies and Nursing staff to the goal of discharging these patients safely to home with the appropriate level of support.

As a Reablement Officer you will be expected to assess our patient's needs and their eligibility for ward-based enablement whilst providing an appropriate assessment upon their level of needs. You will safely support patients in maintaining and improving their level of independence where possible and promote activity to prevent deconditioning.

You will work alongside therapy staff, patients, family and carers to best meet our patient's needs and assess the level of support that is required for patients to return to a domestic setting. You will also work closely with the clinical multi-disciplinary team and our Transfer of Care Hub to ensure that patients are discharged as soon as possible.
