We are seeking an 8a Operational Manager for our Integrated Discharge Team based at Wythenshawe and Trafford General Hospital. WTWA IDT are looking for the right person with the right values, skills and energy, a person who is resilient, holds the expertise and enthusiasm to help us provide our patients with the best possible outcome for their onward journey from hospital, this involves helping us achieve our Home First Approach to improve the quality of life, and independence for the communities we provide the Discharge to Assess model too.

The post holder is responsible for managing a team of nurses and pathway coordinators who are responsible for the assessment and discharge planning of patients with complex needs ensuring the team provide a high quality, effective specialist nursing service to patients.

The post holder will possess expertise in patient flow and discharge processes, change management and the provision of educational support to patients, careers, and colleagues across organisational and professional boundaries.

This person will need excellent IT skills and be able to develop and maintain an environment of trust, diversity, and inclusion within the team, have an agile personality with interpersonal skills with a dynamic approach to complex issues.



The post holder will support the Director of Patient Flow and Head of Patient Flow with the leadership and operational management of the integrated health and social care discharge planning team and its service supporting in the efficient and effective management of resources to deliver in improved performance, promote quality improvement and lead on specific projects and programmes.

The post holder will deputise for the Director of Patient Flow and Head of Patient Flow as required.



You will take a lead role supporting, implementing pro-active discharge planning, working closely with primary care, social care, and community colleagues to deliver an effective and efficient discharge process for complex discharges. The remit will be to lead the day-to-day coordination of discharges into the community ensuring all decisions and actions support patient flow, reduce delays and manage risks effectively.



You will be responsible for managing health and social care staff, implementing agreed policies, clinical standards, and monitoring outcomes. You will demonstrate effective leadership skills maintaining a managerial role.



The post holder will ensure that all protocols that are designed for the discharge service are adhered to, effecting a safe and timely discharge into the community.



The post holder will be responsible for the teams’ activities promoting the independence and well-being of adults and ensuring the service delivered is in line with National Frameworks
