Thisis an exciting opportunity for apermanentBand6Stroke Nurse(18hours a week with regular shifts),within the East Berkshire Earlier Supported Discharge team for Stroke Rehabilitation.The posthas an office base atSt Mark’s Hospital,Maidenhead, clinical carewill beprovided in patients’own homeswithinEast Berkshire but some cover to the West of Berkshire will also berequired. Thesuccessful candidate must be a car driver.





In this diverse role the successful candidate will deliverhigh qualitynursing carefor patientswho have been discharged from hospitalfollowing a stroke, related to their individual goalsand needs. The post holder willreceivesupportfor their caseloadfromotherexperienced Band 6 Nurses as well asfromthe Band7 StrokeNurseTeam Lead.The post holder will be able to work in a cohesive mannerwithand seek support fromother members of thespecialist team,Occupational Therapists, Physiotherapists, Speech and Language Therapists, PsychologistsandTherapyAssistants. The team isforward thinking andthere is a culture of continual improvementto achieve exceptional outcomes for patients.

• You will work closely with other Nurses to complete holistic nursing assessmentsas well asplan, deliver and evaluateevidence-basedinterventionsto achieve the best outcome for patients.
• Develop knowledge and clinical competence within stroke and rehabilitation and utilise this to deliver excellent clinical care with the support of Senior Nurses and theMulti-disciplinary Team.
• Use excellent communication skills to engage and motivate Stroke Survivors toengage in rehabilitation andadhere to secondary Stroke prevention advice, as well asliaise with relevant professionals withinthe team and in wider services such as GP practices,StrokeConsultantsand the Stroke Association
• To complete relevantadministrativeduties such asaccurateand concisedocumentationof notes, reports and data recording as required by the role.
• Youwillplay a rolein co-ordinating referralsestablishingsuitability for the service and attending hospital multidisciplinary meetings asrequired.
