An exciting opportunity has arisen for a practice teacher/clinical team leader to join our Intermediate Care Team.
The post holder will be welcomed into a substantial and supportive rehabilitation team. We are looking for a highly motivated and passionate band 7 nurse who can provide clinical expertise in the delivery of nursing care and leadership within the team.
To support the Operations Manager in delivery of this wonderful service and to adopt a supportive culture for teamwork and building. The Intermediate Care Team cares for patients needing rehabilitative/social care within Community Rehabilitation Units throughout Hull and East Riding and through our UCR 2-Day service seeing patients within their own home and preventing hospital admission.
Previous rehabilitation experience would be an advantage.
