Crynodeb
The role represents a unique opportunity to accelerate this work, via co-production with patients and colleagues from across the health and social care landscape and also representatives from our local communities. It will also improve the planning, delivery and evaluation of services that support people entitled to s.117 aftercare services. The role will coordinate and drive work to devise a process for the implementation of Personal Health Budgets (PHBs) for people in receipt of s. 117 after care services. A focus on quality, coproduction and relationships across the stakeholders network are required for this position.
