An exciting role for a pharmacist with a passion for improving patient safety. The pharmacist will be recognised as the Medication Safety Officer for the Trust and will be the lead within the department and across the Trust in driving safety improvements in medicines management and optimisation, ensuring the Trust has safe processes in place and is compliant will all relevant safety guidance. The post holder will be supported by the Medicine’s Optimisation clinical nurse specialist.

To support the Lead Pharmacist for Governance as the pharmacy professional lead on medicines safety within the Trust by:
• Identifying best practice in relation to medication safety and implementing this locally.
• Assessing compliance against standards and implementing strategies to address gaps.
• Working collaboratively with pharmacy, medical and nursing staff to implement local and nationally agreed medication safety guidance.
• To review incident trends, cascade learning and identify solutions as part of multidisciplinary group.
• Carry out the role of the Medication Safety Officer for the Trust.
• To act as the department’s audit lead for medication safety issues, encouraging audit and original research to support safe medication practice.
• Lead on response to service complaints and ensuring learning is shared from these complaints.
• Practicing as part of the pharmacy team, contributing to the provision of high quality and consistent pharmaceutical care within the Trust.
• Providing directorate level support for a speciality.
• Maintain an awareness of national medication safety issues and initiatives and ensure the Trust is fully informed.
• Attend the Trust’s mortality review group.
• Attend Trust patient safety incident, review and learning group

The Royal Cornwall hospital pharmacy department is a forward thinking and friendly team with full deployment of electronic prescribing and administration in the inpatient and outpatient setting and fully automated dispensing. The data held within the EPMA system gives this post-holder an excellent opportunity to be a leader with ‘technovigilance’ and how acute Trusts can use this data to proactively improve patient safety and outcomes.
