There is a responsibility for caring for the ward patients and the Junior Clinical Fellow (JCF) is expected to support the Foundation Year1 trainee. The JCF will join the morning ward round with the FY1, Senior Clinical Fellows and Consultants. Emergency referrals from the Emergency Department are covered by the on call JCF. There is also an Urgent Care Centre that facilitates the efficient management of patients who need emergency attention that the emergency ward JCF will help support in. The appointee will have also have responsibility in supporting the general surgery team with assisting in theatres and clinics as appropriate. The successful candidate will participate in on-going audits and monthly presentations of our complication rates at the General Surgery M&M meeting. The post is intended to offer appointees, where possible, equivalent opportunities for personal development as ie a study leave entitlement and allocated study leave budget.

Locums are employed only for unexpected leave, such as sickness. All leave, whether annual or study leave, must be booked at least six weeks in advance, to fit in with the partial booking system which allows patients to choose dates for outpatient appointments. This means that leave must be co-ordinated, and may have to be refused if more than two JCFs wish to take leave at the same time. Leave arrangements must be made in a timely way; it is only possible to carry over 5 days of leave not taken, to the next year.

The timetable for SHOs is based on a 1:12 on call rota (the on calls are shared between the general surgery, breast and urology teams). Within this rota SHOs are allocated CEPOD theatre and Trauma theatre and Emergency ward support on a 1:8 rolling pattern. The department rota is arranged on a weekly basis by the current Senior SpR. You will be allocated to a number of Theatre, colorectal or upper GI ward rounds and Outpatient sessions that will vary each week.

When on-call you are not committed to any elective duties and will be involved with the emergency team lead by the Consultant of the Week.

The General Surgery Department at West Middlesex University Hospital is dedicated to teaching and training the junior medical staff including medical students from Imperial College.

Daily handover meetings are held at 8 – 8.30 am, which include all those coming on or off duty attend.

Clinical:

The successful candidate will be expected to help the Specialist Registrars in the administration of the day to day running of the firms and to supervise, help and instruct the pre-registration house officers wherever possible. Involvement in the OP clinics may be required.

The successful post holder will be expected to assist and manage the in patients as well as taking referrals from ED and other departments.

The candidate will also be expected to be part of the on call rota for General Surgery, Colorectal and Urology.

Teaching:

The senior staffs undertake teaching of the Senior House Officers. Senior House Officers would be expected to teach medical students and contribute towards various teaching programmes on the unit.

Research:

Several clinical research projects are in progress and ample opportunities exist for presentation and publication of case reports to the learned societies.

Audit:

JCFs will be encouraged to carry out audit projects with the support of audit department, and must attend the regular department audit sessions.

Administrative:

Offices and secretarial services are available.

Study leave and Training:

This post is not recognised by the London Deanery as a training post but facilities for study leave exist. There is a good recently expanded Medical Library situated in the Postgraduate Centre and Department Library facilities are also available.

A wide variety of postgraduate medical education courses and lectures are available locally including:
• Courses of lectures arranged by clinical tutor
• Weekly physician clinical meetings
• Weekly surgeon meetings, including clinical pathological conferences
• Orthopaedic tutorials
• Regular Audit meetings
