Crynodeb
Specialist Palliative Care Nurse (Community). The post holder will initially be based at the location specified at interview. However, the Trust reserves the right, with consultation, to relocate post holders to any base within the Trust in line with service requirements To manage a caseload of patients with complex palliative care needs, to include physical symptoms and psychological support. To carry out thorough holistic assessments, plan, implement and evaluate treatment plans appropriately. Involve the patient and families/carers in any decisions made regarding care. Liaise closely with other healthcare professionals, attend GSF's and MDT meetings as appropriate. To support families and carers, signposting to other services appropriately. To work as part of the MDT and act as a resource for other professionals who may require specialist advice in managing palliative patients in the community setting. This is just a brief outline of the role.
