Summary


An exciting opportunity has become available to join our Inpatient Rehabilitation Multi-Disciplinary Team at Meadow Court, in the role of our CHC Clinical Nurse Assessor and Discharge Facilitator.

We are seeking a motivated and forward thinking registered qualified nurse to support the flow of patients through our inpatient rehabilitation unit, whilst ensuring all patients discharged onto the D2A nursing home pathway are assessed against the criteria for NHS Continuing Care.

The successful candidate will need to be able to demonstrate knowledge, skills and experience of person-centred assessments. The role will involve working within a multi-agency environment to support the assessment and review process for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC).

Experience undertaking mental capacity and best interests' assessments is advantageous and an understanding of deprivation of liberty safeguards and the Court of Protection would be beneficial to the role. You will also need to be knowledgeable in report writing to ensure you can confidently complete a health needs assessment, personal care and support plan and the CHC decision Support Tool.



You will need to be competent in using Microsoft Word, Excel and Outlook and also be confident in learning new systems for recording and report writing.

The successful applicant will be required to work both autonomously and collaboratively in a team and be able to adapt to a rapidly changing environment. This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process, new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

High Cost Area Supplement - London

This post also attracts payment for High-Cost Area Supplement of 15% of the basic salary (minimum payment of £4,313 and a maximum payment of £5,436).
