The Paediatric Diabetes Specialist Team are looking for an experience Paediatric Diabetes Specialist Nurse to join their team for a 1 year secondment.

We are looking for a dedicated and enthusiastic person to join our forward thinking and innovative diabetes team. The post holder will provide clinical management of diabetes in childhood and adolescence to enable Children and Young People (CYP)with diabetes to reach their optimum level of health, independence, social and psychological/emotional well-being,

You will be involved in empowering newly diagnosed CYP and their families to self- manage a complex condition by supporting them with structured education. They will recognise and adapt to each child and family's individual circumstances and requirements and continue to provide support until the age of 19yrs, when they will be supported to transition to the young person's clinic.

You will have knowledge of the different types of treatment available to support the management of diabetes, including close loop insulin pumps, and recognise when a CYP may benefit from these.

Whilst working as part of a multi- professional team, the post holder will be involved in continuous improvement for care provided in hospital and community settings such as schools. This will be supported by attending regular network meetings to ensure the care provided follows the local and national guidelines.



A Paediatric Diabetes Specialist Nurse supports children, young people and their families with their ongoing management of diabetes, within both hospital and community settings. They will work as part of a multi- disciplinary team, including consultants, dietitians and psychologist ensuring that current, evidence based information and advice is available.

You will be responsible for ensuring the planning, delivery and evaluation of care to meet the personalised health and wellbeing care needs of patients, They will have an active role in the delivery of structured education to families, schools, nurseries and other agencies.

You would be joining a team of experienced and forward-thinking Specialist Diabetes Nurses, Consultant Paediatricians with a specialist interest in Diabetes , Specialist Dietitians, psychologistand Administrative staff alongside working within a large supportive Children's MDT.

Our Specialised Nursing Team provides specialist advice and guidance to staff working across many areas in and out of hospital to ensure that Children and Young People (CYP) can remain out of hospital, where clinically possible. This covers a busy general paediatric department that includes inpatients, outpatients, MDT clinics, group education sessions and can encompass community visits to the CYP’s home and school. We are well established within the children’s and young people’s division in the Hospital and work closely with all members of the MDT.

Duties will include, but not be limited to:
• To provide specialist diabetes advice, support and education for the child/young person 0-19years, and their families/carers upon diagnosis.
• Provide ongoing care and establish professional trusting relationships with individual patients, families/carers. Support the child/young person and family with their ongoing management of diabetes, within both hospital and community settings.
• Support during planned transition to adult services.
• Have an active role in the delivery of structured education to schools, nurseries and other agencies.
