Description
An exciting opportunity has become available for a qualified Social Worker to join the Fostering Service as an Assessment social worker. This would include the completion of Special Guardianship assessments and Connected Person Assessments including conversion assessments for those carers pursuing a Special Guardianship arrangement. This is an exciting time to join Cardiff, which is an Authority on the implementation journey that uses strengths based practice in their approach to working with children and their families.

We are currently seeking a passionate and dedicated individual to join our team as a Cardiff SW in the Social Care industry. In this role, you will be responsible for providing exceptional support and care to patients in our hospital.

As a Cardiff SW, your main responsibilities will include assisting patients with daily activities, ensuring their comfort and safety, and maintaining accurate and up-to-date patient records. You will work closely with healthcare professionals to develop and implement care plans, and provide emotional support to patients and their families. This is an exciting opportunity to make a real difference in the lives of those in need.

Responsibilities
Assisting patients with daily activities such as bathing, dressing, and eating
Monitoring patients' vital signs and reporting any changes or concerns to healthcare professionals
Providing emotional support and companionship to patients
Administering medication as prescribed by healthcare professionals
Ensuring patients are comfortable and facilitating their needs
Documenting patient information and maintaining accurate records
Collaborating with healthcare professionals to develop and implement individualized care plans
Requirements
A valid certification in social work or a related field
Excellent communication and interpersonal skills
Strong empathy and ability to provide emotional support
Proven ability to work in a fast-paced, high-pressure environment
Sound knowledge of medical terminology and procedures
Ability to maintain patient confidentiality and adhere to ethical guidelines
Strong organizational skills and attention to detail

Hours Per Week: 37.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £30.50
Location: Cardiff, Cardiff County
