As a Nursing Associate, you will work under the supervision of and in partnership with the Senior Nurses to provide and deliver a high standard of nursing care. You will contribute to the ongoing assessment, providing and monitoring care based on evidence and working in partnership with the multi-disciplinary team. Key responsibilities Provide compassionate, safe and effective care and support to patients Monitor the condition and health needs of patients within their care on a continual basis In partnership with patients, families and carers, ensuring appropriate escalation to a registered nurse or other professional when required. Support patients to improve and maintain their mental, physical and behavioural health and well-being. Promote comfort and well-being by ensuring that patients personal and social needs are met and be able to care for a patients nursing needs. To safely undertake the following procedures to meet the patients nursing needs to include; undertaking routine procedures including venepuncture, ECG recording, recording monitoring vital signs, wound care, nutrition, hydration, foot care, skin integrity. Carry out immunisations in accordance with local and national PSDs (once additional training has been competed). Provide general and specific health screening, including cervical cytology (once additional training has been completed) to the practice population (within agreed protocols), referring to the General Practitioner when necessary. Identify and report changes in a patients condition to a registered nurse or other professional. Prioritise and manage own workload, recognising where elements of care can safely be delegated to other colleagues, carers and family members. Provide and receive sensitive information concerning patients medical conditions. Work with registered nurses and other professionals and engage with other agencies and community teams to ensure a safe, personal approach for patients. Maintain accurate, contemporaneous patient records in line with practice policy and the NMC Code. Provide effective evidence based care for service users under the supervision of registered nurses and other professionals in order to perform non-complex activities, in line with clinical governance and professional standards. Support registered nurses to enable them to be able to focus on the more complex clinical care, Care for all practice patients including individuals with dementia, mental health conditions, and learning disabilities May include domiciliary visits
