We are excited to announce a newly created role within the Medway Low-Intensity Support Team (LIST) for aRegistered General Nurse. In this important role, you will play a key role insupporting the physical well-being of adults with a functional mental illness.

About the Role:
• Work collaboratively with mental health professionals and service users to manage and improve physical health outcomes.
• Provide education and support to service users on healthy lifestyle choices, including nutrition, physical activity, and preventative healthcare.
• Liaise with other healthcare providers, such as GPs and community nurses, to ensure coordinated care for service users.

About You:
• You are a qualified healthcare professional with experience working with individuals with mental health conditions.
• You are passionate about promoting physical well-being and integrated healthcare.
• You are a skilled communicator and team player with the ability to build rapport with service users and other professionals.

• To ensure that physical health is viewed on path with mental health needs.
• Encouraging and collaborating with patients to raise awareness of common physical health conditions that may occur in people with a serious mental illness. Supporting them to overcome barriers to annual routine physical health checks, e.g. blood tests and ECG.
• Supporting the team to achieve physical health KPIs
• Responsible for managing physical health care plans and goals
• Responsible for building relationships and sharing information with PCNs in Medway to support better physical health in our patient group
