As the organisational expert act as ambassador in the promotion of high quality, cost effective and evidence-based medicines management. Ensuring appropriate handling of medicines across the organisation and system/ partners include the provision of specialist advice, evidence-based advice on all aspects of medicines and their use. To ensure compliance with all legal and regulatory medicines handling requirements, utilising complex knowledge of medicines management. To lead on the medicines management optimisation agenda for the organisation, promoting safe, effective, and legal medicines use and handling at all levels of the organisation, and across all clinical teams, partners and commissioners. To be the lead medicines professional within the Quality and Safety team, ensuring the delivery of core medicines governance work in terms of Medicines safety, Incidents/risks, Audits, Policies/SOPs, Documentation, advice. As the organisational expert on medicines management, undertake the role of Medicines Safety Officer by planning, prioritising, and reviewing medicines management activity across CityCare and where it interlinks with its partners ensuring safe management of medicines. To manage the medicines management team workload and pharmacy technician led activities. To manage the organisations medicines team workload, including service development work and projects within specific clinical teams. To work with managers and clinical teams regarding medicines processes and policy, balancing risks and operational restrictions. To provide advice to managers and clinical teams, considering service To be the organisations expert on Controlled drugs, to liaise with the Controlled Drugs Accountable Officer and participate in the CD LIN. Utilising expert knowledge to lead on the delivery of effective and evidence-based medicines information/communication, education, training and competency assessments in a variety of forms by fully engaging with relevant groups including the ICB, Partners and CityCare to ensure all relevant clinical teams and services are working to the same standards. To be the development pharmacist for the development and review of PGDs for the organisation and provide professional input into the assurance and approval processes for these. Ensuring all medicines management activity falling within CityCares remit including anything connected with partners meets the standards required of PGDs To provide support to all the NMPs within the organisation, from a clinical, professional, legal and operational perspective, including participation in NMP education forums. To provide prescribing analysis at an individual, team and organisational and partners level, and monitor this data in terms of formulary compliance and prescribing budgets/medicines costs. To contribute to the education, training and competency assessment of NMPs Key Responsibilities The post holder will lead on all medicines safety, optimisation and handling processes within the organisation as detailed below. Medicines leadership Medicines optimisation work planning To lead on the development of the Medicines Optimisation agenda within the organisation, and to manage the overall medicines workstreams for the organisation. To provide strong leadership on medicines to all elements of the organisation, including the Quality and safety team, Workforce development, all clinical services in adults and childrens divisions. To take the lead on medicines safety initiatives and planning. Represent CityCare at local, regional or national medicines meetings. To represent Citycare at various local, regional and national medicines meetings as required, such as the regional MSO meetings, CD LIN, Area Prescribing committee. Governance Medicines safety officer To undertake the responsibilities and duties of the Medicines Safety Officer as stipulated in current guidance. To liaise and network with other MSOs in similar organisations. Facilitate the implementation of recommendations in national guidance e.g.: NICE guidance, NPSA alerts and local clinical guidelines and co-ordinate this activity across the locality. Advise and monitor adherence to these guidelines. Incidents/Risks To support teams when medicines incidents occur, with advice and guidance. To collate and categorise all medicines incidents and produce reports summarising themes and actions required for various stakeholders. To liaise with Quality and safety colleagues and clinical teams to support changes to clinical practice and policies/procedures following on from incidents, including the dissemination of learning. To collaborate with the CityCare Clinical Governance leads and structure to ensure rational prescribing and best practice re protocols and guidelines relating to medicines in the local health community. Audits To manage a programme of medicines audits across the organisation, including writing audit plans and reports and supporting action plans. Develop, undertake and supervise medicines management audits within services and within specified locations to encourage them to critically appraise their current practice and adopt best practice standards To manage medicines handling audits within health centre bases. Policies / SOPs To facilitate the development of consistent standards of practice and polices in line with legislation, best practice, local and national guidelines. To participate in multidisciplinary groups producing written guidance To liaise with secondary care to promote and ensure seamless care of patients across the interface. To support the business and clinical teams to develop suitable clinical processes, care plans and templates within SystmOne Advice To respond to queries and incidents related to medicines, and offer suitable advice and support To advise on the safe handling of medicines in a variety of clinical services To advise on the legal and professional handling of medicines in a variety of services, and during periods of service development and change Medicines team workload management Medicines technicians led services. To manage and support the technicians in their work on a regular basis. This will include work prioritisation, ongoing support, supervision, and monitoring of the technicians work. Lead on the development of new medicines related services, including development of related guidance and protocols. Medicines projects within other clinical services, and service development To provide clinical advice for the development of new services, or changes within existing services, related to medicines handling and safety. To advise on medicines processes across multiple clinical services, across adults and childrens, urgent care centre, including out of hours. Medicines information and training Newsletters / alerts To produce newsletters and medicines alerts in response to local, regional and national medicines Intranet To manage the content and structure of the Medicines management intranet to make this a useful resource for all staff within the organisation. Medicines information enquiries To respond to medicines information queries of a clinical nature, either general or related to specific patients. To respond to enquiries regarding the handling of medicines for service development purposes. Training Lead on the development and provision of evidence-based medicines training for health and social care staff within the organisation, via face to face, written or online means. Lead on the development of suitable competency assessments for health and social care staff within the organisation PGDs Development group pharmacist To act as the development pharmacist in collaboration with nursing and medical colleagues in the development of PGDs for use within the organisation. This will include clinically reviewing existing PGDs, assessing the need for new PGDs, and writing new PGDs as required. Active member of regional PGD group To act as the pharmacist representative for the organisation at the PGD assurance and authorisation group. This will include presenting PGDs developed by City care and reviewing and providing feedback on PGDs developed by other organisations. The post holder will also participate in the governance activities of this group, contribute to the development of templates, policies and processes, and in the work planning and prioritisation. NMPs NMP support role To contribute to the development of prescribing policy and attend/organise meetings relevant to role. To provide advice to NMPs on specific prescribing and clinical issues To support the development of robust tools to record and monitor prescribing in SystmOne NMP education forums Post holder will be a key member of the NMP working group to develop a suitable NMP training programme. Post holder will deliver training relevant to NMPs at this regular NMP education session. Prescribing analysis Post holder will support the clinical analysis of Citycares prescribing data at an individual, team and organisational level. Post holder will support managers to interpret the clinical prescribing data for their team. Post holder will review the prescribing data in relation to local formulary compliance and in terms of medicines safety metrics. Budgetary considerations Post holder will monitor the prescribing costs generated on an organisational and service level, suggest medicines changes which would reduce prescribing spend. The post holder will manage the costs associated with medicines stocks held across the clinical services, including PGD medicines.
