This exciting opportunity will include working within a speciality team to manage patient prescribed Domiciliary Oxygen, to be responsible for the day-to-day co-ordination of the service and data management of the St Helens Oxygen Service.

The post holder will be proficient and competent to work across professional disciplines, referring clients for assessment and treatment or to other agencies as required. To monitor and instigate reviews which will be both carried out virtually in the office and in the patient’s homes to ensure all patients receive appointments in a timely fashion as required.

The post holder must have key computer skills including the ability to use excel spreadsheets to interrogate oxygen data, raising invoice queries to generate refunds and to plan nursing colleagues’ workload.

It is essential that the post holder exercise initiative commensurate with the role and that confidentiality is always maintained.

The care coordinator will assist in the identification of health needs and participate in the delivery of St Helens Home Oxygen Service (HOS-AR) under the supervision of a Registered Practitioner. Supervision may be direct/indirect depending upon competency/complexity of work.

The post holder will continue to develop and undertake a range of delegated nursing tasks across primary and secondary care and will report to the Specialist Nurse.

An Enhanced DBS Certificate with the checking of Adults’ Barred Lists is required for this post.


• Will be the first point of contact for all Domiciliary Oxygen Service queries from patients, Clinical Commissioning Groups, Oxygen providers and staff within Primary and Secondary care
• Responsible for supporting the operational and administrative functioning of the Home Oxygen Service under the support of the Oxygen Team Leader
• The post holder will be responsible for referral management and will be able to demonstrate their professional skills and competence in leadership, planning and coordination.
• Accountable for reviewing whether a referral to the service meets the established referral criteria and is compliant with referral guidelines.
• Responsible for ensuring appropriate information is readily available to members within the Multidisciplinary Team (MDT) and that this information is regularly updated.
• Heavily support the discharge process of Oxygen patients by working collaboratively with Respiratory Nurses across primary and secondary care, oxygen providers and the Medicines Management Team.

Key Duties Continued
• Liaise closely with MDT team members involved with the Oxygen Service and related departments and will provide admin support for the Oxygen Service.
• Responsible for arranging the maintenance and timely service of oxygen equipment.
• Liaise with clinicians, Oxygen Nurse Team Leader, and Respiratory Nurse Specialist regarding coordinating oxygen assessments across primary and secondary care and clinic administration.
• Work collaboratively with health professionals and other agencies to ensure patient needs are met and devise an individual plan for discharge planning –from admission to discharge. This will also involve working with other members of the MDT group i.e. General Practitioners, Community Matrons, Palliative care teams, Discharge Planning Team, OT, Physio and Medical Social Workers.
• Triage all referrals, reviewing complex clinical information as necessary and informing the appropriate speciality nursing colleagues.
• To work closely with clinicians to enable a maximization of clinical effectiveness within referral management and to work within the clinical governance framework.
• To be the main link and key contact between the Nursing Staff and other members within the MDT team and assist in developing appropriate care packages and support for the individual patient based on their needs.
• Deliver clinical and therapeutic care within set protocol.
• Hold a manageable caseload of housebound and/or palliative patients undertaking a holistic assessment as and when required under the supervision of the Oxygen Team Leader
• Identify deteriorating or unwell patients and refer on to the Specialist Nursing Team
• Organise and prioritise time effectively.
• Provide clinical and administrative support to the St Helens HOS-AR in the delivery of care to patients in the clinic venues and community setting.
• Ensure adequate stock ordering and stock rotation to ensure service delivery.
• Ensure care is documented accurately and in a timely manner as per Trust Policy
• Undertake initial and follow up clinical assessments of oxygen patients independently and/or with the support of a Registered Nurse, within either a clinic or domiciliary setting.
• Perform initial and routine home safety checks, and complete a risk assessment, raising any concerns to the Multidisciplinary team members and/or taking immediate action depending on the severity of the situation.
• Perform ambulatory oxygen assessments within a supervised environment, determining the need for this form of therapy and educating the patient and/or carer accordingly.
• Liaise with the Oxygen Team Leader regarding completion of the Home Oxygen Consent and Order Forms, ensuring the most appropriate equipment is selected based on individual patient need.
• Maintain clear, concise, and legible documentation adhering to prescribed standards and protocols.
• Use IT systems for relevant taskse.g., accessing patient records and results, requesting investigations, data collection etc.
• Obtain verbal consent in accordance with local policy prior to undertaking any procedure.
• Maintain a patient orientated approach towards care delivery.
• Undertake a designated range of patient assessments (according to protocol)
• Plan, deliver and promote high standards of evidence-based care (in conjunction with relevant professionals where appropriate)
• Implement and evaluate individual care plans, utilising relevant outcome measures.
• Undertake a designated range of interventions (according to protocol) to promote functional independence.
• Recognise the need for and undertake appropriate referrals to Specialist Nurses/Practitioners/Services within the NHS, independent and voluntary sectors.
• Promote and provide health education specific to respiratory care and in line with national and local policies and identify patient needs, coordinating referrals as required.
• Undertake, monitor, and record the following basic observations:

Temperature, blood pressure, pulse, respiration, saturation level

Peak flow

Carbon monoxide monitoring

Complete Malnutrition assessment and weight monitoring for patients

Falls assessments.

Waterlow score pressure sore risk assessment tool low assessment.

• Ensure any readings outside of normal parameters are highlighted and/or discussed appropriately within the clinical tea.
• Foster and promote the concept of multi-disciplinary team (MDT) working amongst colleagues both within the hospital and community setting to deliver the best possible all-round care to the patient.
• Work collaboratively with the multi professional team both within the Trust and community to ensure a coordinated approach to meeting the needs of the patients and carers.
• Establish and maintain effective communication with patients and families/carers, and professionals across health and social services utilising communication and interpersonal skills.
• Demonstrate the ability to maintain a comprehensive database of patients to collect, collate and evaluate data providing evidence for annual report and service activity ensuring that their storage is secure.
• The post holder will provide education to your patients regarding their disease to assist them to develop self-management skills at home, to prevent hospital admission, in line with national and local guidelines.
• Identify appropriate resources for individual patient needs, referring to appropriate multi-disciplinary teams to prevent unnecessary hospital admissions and any further deterioration.
• Provide expert knowledge to clinical colleagues, patients and families/carers and support as necessary members of the team.
• Attendance at community, primary care, and palliative care MDTs to provide and seek expert advice.
• Assist junior staff to comprehensively access, implement and evaluate programmes of evidence-based care.
• Work autonomously, but within the guidelines, policies, and procedures of the service
• Use the expertise and knowledge of professional colleagues acknowledging the boundaries of practice and liaise with the Oxygen Team Leader
• Establish and maintain continuing relationships with patients/ carers and relatives which are essential to providing effective long-term care.
• Provide psychological care and support to patients/ carers and relatives.
• Provide highly sensitive, complex information to patients, carers, relatives and professionals across health and social services.
• Exercise judgement, discretion, across all aspect of the role within primary and secondary care and the delivery of oxygen clinics, undertake capillary blood gas sampling in community settings in accordance with trust guidelines and protocols, for individual patient needs as per national guidelines under the supervision of the specialist nursing team.
• Demonstrate specialised knowledge to review and monitor the compliance report to ensure clinicians appropriate safe prescribing of oxygen equipment across Part A & part B Home oxygen ordering Form (HOOF) within St Helens locality across primary and secondary care for long term oxygen therapy (LTOT) Ambulatory oxygen therapy (AOT) and short burst oxygen therapy (SBOT)
• Support Primary care clinician to assess and prescribe safely.
• Adhere to trust and service policies and standard operating procedures.
• Establish and maintain continuing relationships with patients, carers and relatives which are essential to providing effective long-term care.
• To communicate with patients and professionals of all levels daily with regards to current oxygen prescriptions and equipment.
• To engage with oxygen provider regarding patient queries, orders and invoice discrepancies.
• To communicate by email to the commissioners monthly with a report following the oxygen invoice.
• To ensure the service oxygen patients database is kept current and up to date.
• To have knowledge and understanding of the oxygen equipment used and/or issued to patients to enable you to discuss its use with the patient and the safety surrounding this equipment (when needed).
• Understanding the oxygen assessment requirements, monitor and arrange appropriate follow- ups at home or in clinic.
• To have knowledge of the Oxygen Service within the UK and know how to source oxygen for patients travelling outside of the UK.
• Post holder will have skill to interrogate data to enable planning of nursing colleague’s workload on a weekly basis (including booking four-week home visits in line with O2 guidelines and focusing on key outcomes such as high O2 users, urgent activity, patients considered to be in O2 providers Escalation Process).
• To undertake training provided by the Oxygen Team Leader and oxygen providers regarding both data and equipment.
• To be able to prioritise workload, arranging home visits in line with national guidance delivering four-week oxygen assessments, management of nurse’s diaries including booking meetings
• To support the team in the promotion of local and national respiratory awareness events
• Where necessary arrange urgent or additional appointments utilising the access plan
• Ensure that RTT targets are tracked and managed in line with Trust policies. Attending any training required to update knowledge on requirements and escalate to Service Managers any barriers identified
