An exciting opportunity has arisen to recruit a Band 7 Discharge Coordinator in the Discharge Team to complement our nursing and admin team.

You will be working as part of a multi-professional Team to support the smooth running and co-ordination of discharge services within the unplanned Care Divisionand preventing unnecessary, avoidable delays for patient discharge.

The post holder will be responsible for facilitating safe and effective discharges of patients from hospital to the community setting through a multidisciplinary approach over 7 days. They will co-ordinate assessments of patients’ needs and support the discharge plan. They will be proactive in identifying concerns or issues that may delay effective discharges and work with the Integrated Discharge Team to overcome and limit these. The post holder will develop, implement, and evaluate a seamless patient transfer of care service, involving the MDT. They act as a specialist and will demonstrate a high level of expertise within the transfer of care (discharge) service, providing specialist advice, education and support to health care professionals and patients. The post holder will work closely with all adult ward teams ensuring appropriate referrals are completed appropriately and made to local authorities.
