To be a point of contact/key worker for patients diagnosed with a breast malignancy at pre-diagnosis, during treatment, in the follow up period and when metastatic disease is diagnosed whether face to face or via the telephone Utilise advanced communication and counselling skills when discussing diagnosis, prognosis and treatment options for metastatic breast cancer with sensitivity and expertise Work autonomously and demonstrate advanced practice by managing a caseload of patients and demonstrate advanced practice in the holistic assessment, physical assessment and non-medical prescribing and evaluation of care promoting the philosophy of person-centred care for patients Implement and manage telemedicine clinics and outpatient nurse led clinics which will include Enhanced Supportive Care clinics To be responsible for maintaining high standards of patient care and contribute to clinical audit To provide psychological support and symptom control for patients diagnosed with both early and advanced breast cancer in a clinic, ward setting and telephone support Use patient feedback to develop relevant support structure including support groups Visit thein-patient ward areas including ED/AAU and review patients, where appropriate, providing guidance and information relevant to their care Provide specialist, expanded roles of diagnostic and therapeutic procedures where necessary Provide specialist advice to support ward staff when patients are admitted ensuring all medical and nursing needs are understood and carried out in accordance with Trust standards and policies Act as a leader, advisor and role model to less experienced nurses, ensuring appropriate care is planned, implemented and evaluated Work collaboratively with the Acute Oncology Team, the Cancer Navigators/Support Workers and the Specialist Palliative Care team to enhance patient care and management.
