Consultant Community Geriatrician
This post will join 2 other Community Geriatricians from LUHFT and 1 from Mersey Care NHSFT and also the team supporting the acute frailty service at the Royal site. The post will seek to develop, strengthen, and provide within the following:
A common model across Liverpool and South Sefton in order to strengthen the provision, enable cross cover, peer support, improved governance, support and measurement.
The aim of provision should be a minimum of 1 WTE to 150,000 population.
Provision of the service from the community and mental health provider will aim to improve integration with relevant other community professionals and dedicated approach to community care.
The focus of direct input will be on providing pro-active care to complex elderly patients through overseeing comprehensive geriatric assessment, care planning, assistance with diagnosis and managing complex medical presentations.
Community geriatricians will be available to provide daily support and direct supervision to community matrons (both domiciliary and care homes) who will be their primary link into the services of Integrated Care Teams in general.
Community Geriatricians will deliver care as part of a MDT approach. Key stakeholders for whom community geriatricians should work alongside and receive mutual support from include:
a. Community Matrons in particular within Integrated Care Teams
b. Intermediate care facilities
c. Care Homes
d. Old age Psychiatry services
e. Frailty units and Geriatrics departments within acute trusts
f. End of Life services
g. GPs
Maintaining links with the respective acute trusts will be secured to ensuring on-going seamless care and positive working relationships. Community geriatricians will have an opportunity to work one day a week on a frailty unit or similar setting.
Provide cover when required for physical health reviews for psycho-geriatrics in-patients.
The current model of responsive (less than 2 week) review from GP referrals within out-patient clinics, domiciliary home visits, care home in-reach will be part of each community geriatricians weekly job plan
