We are looking for a Band 5 Community Physiotherapist to join our team.

The position will be covering the Nottingham North and East area. We have home teams based in Carlton, Hucknall and Arnold, with the flexibility to work remotely and from home on occasion. The successful candidate will work within the NNE Community Therapy Team and support the delivery of services such as Complex Community Rehabilitation, Short Term rehabilitation, Discharge to Assess and Long-term conditions management.
Our passion is to maintain independence and keep individuals at home for as long as they are able, avoiding hospital admission and enabling a return home where admission has been unavoidable.

Working within the integrated team, you will be supported to effectively triage, prioritise, assess and provide holistic interventions for clients referred to the service, within their home environment. You will work closely with health and social care colleagues across primary and secondary care to ensure continuity, effective intervention and maximise rehabilitation outcomes.
You will offer bespoke physiotherapy to adults, the frail and elderly including complex fallers, patients with neurological conditions, complex co-morbidities and orthopaedics.
We deliver services between 8am and 8pm over 7 days. We currently operate shift patterns of 8-4pm/12-8pm on 5/7 days.

Applicants should hold BSc/Diploma in Physiotherapy, HCPC Registration. You will ideally have post qualification experience and recent experience of working with clients with long term conditions, falls, neurology and health care of the elderly conditions.
A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
